目的 探讨肢体静脉畸形伴发关节功能障碍的相关危险因素。方法 2013年1月至2015年8月,收集肢体单纯静脉畸形患者临床和影像学资料,初诊时伴发关节功能障碍者作为功能障碍组,未出现功能障碍者为对照组,对两组患者的资料进行相关危险因素分析。结果 共168位肢体静脉畸形患者入组,其中功能障碍组44例,临床表现为关节屈曲挛缩或伸直受限,以及关节外翻或内翻畸形。单因素分析显示,功能障碍与患者年龄、部位、病灶大小、累及组织、疼痛评分及持续时间有统计学相关性(P<0.05)。非条件多因素Logistic回归分析显示,年龄≤18岁、下肢病灶以及疼痛时间超过1年,是肢体静脉畸形伴发功能障碍的独立危险因素。结论 对于肢体静脉畸形除了早期诊断,更要加强对疼痛的早期控制和有效缓解。
Objective To investigate the determined risk factors of joint dysfunction in patients with limb venous malformations. Methods Patients with venous malformation of limbs were collected from Jan 2013 to Aug 2015. Their initial presenting symptoms were categorized into the dysfunction and control groups. Independent risk factors for dysfunction were identified between the two groups. Results A total of 168 patients with venous malformation of the limbs were identified. Among them, 44 (26%) patients presented as flexion or extension limitation and varus or valgus were enrolled into the dysfunction group. In univariate analysis, age, location, size, involved tissue, visual analogue scale of pain and pain duration were significantly associated with joint dysfunction (Pone year) remained independently associated with joint dysfunction. Conclusion It highlights that the importance of an early and precise diagnosis in limb VMs. Pain relief is also a key approach to prevent joint dysfunction and pain should be under control in early.
