Sommerlad腭帆提肌重建联合带蒂颊脂垫瓣在完全性腭裂修复中的应用

摘要： 目的 探讨一种对上颌骨发育影响较小,且能有效降低腭瘘发生率的功能性腭裂修补术。方法 自2013年起,采用Sommerlad腭帆提肌重建联合带蒂颊脂垫瓣修补38例腭裂患者。结果 所有患者均一期愈合,均无腭瘘发生。术后随访1~2年,患者语音清晰度满意。结论 Sommerlad腭帆提肌重建联合带蒂颊脂垫瓣,可有效降低腭瘘的发生率,远期腭咽闭合良好,避免了裸露骨面对上颌骨生长发育的影响,是一种值得推荐的功能性腭裂修复术。

Abstract: Objective To explore a method of functional palatoplasty for reducing the incidence of palatal fistula without interfering the maxillary growth. Methods From 2013, a total of 38 patients with cleft palate received levator veli palatini retropositioning combined with buccal fat pad flap. Results All patients were healed by first intention, no palatal fistula was observed. All the patients were followed up for 1-2 years, significant improvement in speech intelligibility was achieved. Conclusion Levator veli palatini retropositioning combined with buccal fat pad flap is a useful procedure for cleft palate repair with low incidence of palatal fistula and satisfying rate of velopharyngeal competence. Furthermore, this technique affects growth of maxillary very slightly and is worthy of further application.
