西藏地区保留局部皮瓣联合小剂量放疗治疗耳廓瘢痕疙瘩的临床研究

摘要：

目的　探索在西藏（高原）地区采用保留瘢痕表面局部皮瓣联合小剂量放射性治疗的方法，对耳廓瘢痕疙瘩治疗的有效性。方法　2022 年 4 月至 2022 年 8 月，西藏自治区人民医院整形外科 8 例耳廓瘢痕疙瘩患者，局麻下行瘢痕疙瘩切除，保留局部瘢痕皮瓣，术后联合局部电子线放射治疗。结果　术后随访 3~8 个月（平均 5 个月），8 例患者术区均Ⅰ期愈合，5 例术区平坦、无局部症状 ；2 例局部偶感轻度刺痛 ；1 例患者术后 4 个月始出现术区轻度结节，伴瘙痒，未行特殊处理，自行缓解。结论　在高原地区，采用保留局部瘢痕皮瓣联合放射性治疗，是治疗耳廓瘢痕疙瘩的有效方法。

Abstract:

　Objective　To explore the effectiveness of local skin flap combined with low-dose radiation treatment for ear
keloid in Tibet region. Methods　From April 2022 to August 2022, 8 patients with auricular keloid were treated with keloid
resection under local anesthesia, local scar flap was retained, and local electron radiotherapy was combined after surgery in the
plastic surgery Department of Xizang Autonomous Region People's Hospital. Results　The patients were followed up for 3-8
months (mean 5 months). All the 8 patients had stage I healing. Among the patients, 5 patients had flat surgical site without local
symptoms, 2 cases had slight local tingling, 1 patient developed mild nodules in the operative area with pruritus 4 months after
surgery, which were relieved spontaneously without special treatment. Conclusion　The use of local scar skin flap combined
with radiation therapy can effectively treat ear keloid in Tibet region.
