摘要:

目的 探讨强化本体感觉训练及等速肌力训练对前交叉韧带(ACL)重建术患者康复效果的影响。方法 选取 2020 年 1 月至 2022 年 12 月在我院诊治的 ACL 重建术患者 82 例,随机分为 2 组,每组各 41 例。对照组给予常规训练,研究组给予强化本体感觉训练及等速肌力训练。比较两组训练前后的膝关节本体感觉功能、等速肌力、关节疼痛程度及膝关节稳定性,以及康复效果。结果 训练后,两组患侧 15 ° 重现差值、患侧 45 ° 重现差值、患侧 75 ° 重现差值较训练前均降低(P<0.05),且研究组明显低于对照组(P<0.05)。训练后,两组伸屈膝相对峰力矩(PT/BW)、屈膝峰力矩值(PT)、伸膝 PT、腘绳肌与股四头肌的肌力比值(H/Q)均较训练前升高(P<0.05),且研究组明显高于对照组(P<0.05)。训练后,两组视觉模拟评分法(VAS)评分、Lachman 试验胫骨前移较训练前均降低(P<0.05),且研究组明显低于对照组(P<0.05)。研究组康复优良率为 90.24%,明显高于对照组的73.17%(P<0.05)。
结论 强化本体感觉训练及等速肌力训练应用于 ACL 重建术患者,可有效改善膝关节本体感觉功能、等速肌力,降低关节疼痛程度,提高膝关节稳定性和康复效果。

Abstract:

Objective To explore the effect of strengthening proprioception training and isokinetic muscle strength training
on the rehabilitation of patients undergoing anterior cruciate ligament (ACL) reconstruction. Methods From January 2020
to December 2022, 82 patients with ACL reconstruction were randomly divided into two groups: 41 cases in the control group
and 41 cases in the research group. The control group was given routine training, while the study group was given intensive
proprioception training and isokinetic muscle strength training. The knee proprioception function, isokinetic muscle strength,
joint pain degree, knee stability, and rehabilitation effect were compared between the two groups before and after training.
Results After training, the 15 ° recurrence difference, 45 ° recurrence difference, and 75 ° recurrence difference on the
affected side of the two groups decreased compared to before training (P<0.05), and the study group was significantly lower than
the control group (P<0.05). After training, the relative peak torque of knee extension and flexion (PT/BW), peak torque of knee
flexion (PT), PT of knee extension, and the ratio of hamstring muscle strength to quadriceps strength (H/Q) of the two groups
were higher than those before training (P<0.05), and the study group was significantly higher than the control group (P<0.05).
After training, both visual analogue scale (VAS) scores and Lachman test tibia anterior displacement decreased compared to
before training (P<0.05), and the study group was significantly lower than the control group (P<0.05). The excellent and good
rehabilitation rate of the research group was 90.24%, which was significantly higher than the 73.17% of the control group (P<0.05).
Conclusion The application of enhanced proprioception training and isokinetic muscle strength training in patients undergoing
ACL reconstruction can effectively improve the proprioception function and isokinetic muscle strength of the knee joint, reduce
the degree of joint pain, and improve the stability and rehabilitation effect of the knee joint
