在MIPPO技术下应用国产LCP治疗股骨远端脆性骨折

Application of Domestic LCP with MIPPO Technique in the Treatment of Distal Femoral Fractures

摘要: 目的 探讨应用国产锁定加压钢板(Locking compression plate,LCP)经皮置入治疗股骨远端脆性骨折的疗效.方法 2011年1月至2015年11月,应用国产LCP经皮置入治疗股骨远端脆性骨折18例,对于有关节内骨折者可仅切开关节部直视下复位,部分需要拉力螺钉固定,使关节面解剖复位;对于股骨髁上骨折则单纯应用MIPPO(Minimally invasive plate percutaneous osteosynthesis)技术行间接复位,锁定钢板固定.结果 18例术后随访10~21个月,平均(15.5±5.2)个月.骨折均愈合,平均愈合时间(14.5±2.2)周,平均完全负重时间(18.5±3.2)周.切口均Ⅰ期愈合,骨折端复位满意,未见术后感染、内固定物断裂、骨不连、深静脉血栓、肢体短缩、膝内翻畸形等并发症.根据Neer评分对膝关节功能进行评分:优13例,良5例,优良率100%.膝关节最终活动度:A3型骨折为124°~135°(130°±6°),C2型骨折为85°~135°(111°±25°),C3型骨折为84°.结论 国产LCP经皮置入治疗股骨远端脆性骨折安全有效,能满足基层医院临床需求.

Abstract: Objective To evaluate the efficacy of percutaneous plate fixation with LCP (locking compression plate) in the treatment of distal femoral fractures. Methods From January 2011 to November 2015, 18 cases of distal femoral fractures were treated with LCP transdermal plate technique. The fractures of intra-articular were only reset under the incision of the joint, and some of them needed to be fixed with the tension for anatomical reduction of the articular surface. The femoral supracondylar fracture was treated with MIPPO (minimally invasive plate percutaneous osteosynthesis) technique and fixed with the plate. Results All cases were followed up for 10~21 (15.5±5.2) months. Fractures were healed at an average time of (14.5±2.2) weeks, and the average load-bearing time was (18.5±3.2) weeks. All the incision were healed by first intention and fracture reduction was satisfied. No postoperative infection, internal fixation fracture, nonunion, deep vein thrombosis, limb shortening, knee varus and other complications were observed. The knee function was graded according to the Neer score: excellent in 13 cases, good in 5 cases, excellent and good rate was 100%. The knee join movement degrees: A3 fracture was 124 °~135 ° (130 ° ±6 ° ), C2 fracture was 85 °~135 ° (111 ° ±25 ° ), C3 fracture was 84 ° . Conclusion Percutaneous implantation of plate technique combined with domestic LCP is a safe and effective method for the treatment of distal femoral fractures. It can meet the clinical needs of primary hospital.
