Abstract: Mesenchymal stem cells (MSCs) are widely used in the field of repair and regeneration because of their numerous biological effects. However, MSC cell therapy for chronic wounds has a number of drawbacks, including a limited ability to homing living cells, a lower survival rate, and the possibility of malignant differentiation. Recent advances have been made in the study of MSCs secretome, and cell-free therapy based on MSC secretome is considered to be a novel strategy for enhancing MSC bioavailability to treat chronic wounds. In this paper, the benefits of MSC secretome therapy, the mechanism by which the secretome acts on the wound, preconditioning methods to improve the secretome therapeutic potential were all discussed, and the research status of MSCs conditioned medium and MSCs extracellular vesicles was summarized, laying the groundwork for future clinical trials.
