摘要:
不可切除肝外胆管癌和壶腹癌恶性程度高、预后较差。目前主要的姑息性治疗为胆道内引流或外引流术,可改善病人生活质量,但对肿瘤本身无治疗作用。内镜下光动力治疗(photodynamic therapy, PDT)是近年来兴起的一项微创治疗胆道肿瘤的新技术,可局部杀灭肿瘤细胞并引发全身免疫反应,可延长病人生存期,临床应用前景广阔。随着PDT分子机制研究的不断深入,以及与化疗、放疗、免疫治疗等的联合应用,内镜下PDT在杀灭原发肿瘤、肿瘤降期及复发肿瘤的治疗等方面取得了显著的疗效。本文就内镜下PDT不可切除肝外胆管癌及壶腹癌的现状进行了分析并展望。
