动力髋螺钉(DHS)和空心螺钉内固定与PFNA内固定治疗老年股骨转子间骨折的比较

摘要: 目的 比较分析动力髋螺钉(DHS)+空心螺钉内固定,与PFNA内固定治疗老年股骨转子间骨折的临床疗效。方法 回顾分析2014年6月至2016年9月收治的80例老年股骨转子间骨折患者,其中采用DHS+空心螺钉内固定治疗的患者40例(DHS组),PFNA内固定患者40例(PFNA组)。所有患者术后随访6个月,比较两组患者一般临床资料、手术情况、住院天数、Harris评分以及术后并发症情况。结果 (1)两组患者在性别、年龄以及骨折分型方面无统计学差异;(2)PFNA组患者平均手术切口长度、手术时间、术中出血量、术后引流量、下地负重时间、骨折愈合时间均明显优于DHS组;(3)两组患者在Harris评分以及术后并发症方面无明显差异。结论 PFNA内固定法更适合老年股骨转子间骨折的患者,值得临床推广应用。

Abstract: Objective To compare the clinical efficacy of DHS plus hollow screw and PFNA internal fixation in the treatment of intertrochanteric fractures in the elderly. Methods From June 2014 to September 2016, 80 elderly patients with intertrochanteric fractures admitted to our hospital were retrospectively analyzed. Among them, 40 cases were treated with DHS plus hollow screw (DHS group), while the other 40 cases were treated with PFNA internal fixation (PFNA group). The clinical data, operation conditions, hospitalization days, Harris score and postoperative complications in both groups were analyzed and compared. Results ①There was no significant difference between the two groups in terms of gender, age and fracture classification. ② The average incision length, operation time, intraoperative blood loss, post-operative drainage, weight-bearing time, fracture healing time of the cases in the PFNA group were significantly better than those of the cases in the DHS group;③There was no significant difference in the aspects of Harris score and postoperative complications between the two groups. Conclusion PFNA internal fixation is more suitable for elderly patients with intertrochanteric fractures, which is worthy of further promotion.
