团队学习联合病例教学用于甲状腺外科教学的前瞻性研究

A prospective study on team-based learning combined with case-based learning in training thyroid surgery of medical intern

摘要: 目的 探究以团队为基础的教学方式(team-based learning, TBL)联合以病例为基础的教学方式(case-based learning, CBL)在甲状腺外科实习医师教学的应用价值。方法以2020年11月至2021年6月在瑞金医院甲状腺外科临床实习的94名实习医师为研究对象进行前瞻性研究。以实习时间节点分组,在2020年11月至2021年2月临床实习46名实习医师为对照组(n=46),在2021年3月至6月48名实习医师为研究组(n=48)。对照组行传统教学方式,研究组行TBL结合CBL联合教学方式。比较两组学习的基础理论知识、基本操作能力及危机处置能力考核成绩。结果 研究组实习医师的基础理论知识成绩高于对照组(t=12.610, P<0.05)。基本操作能力(t=1.566, P>0.05)和危机处置能力(t=1.313, P>0.05)成绩两组间差异无统计学意义。在危机处置能力中,研究组的团队协作能力(t=3.610, P<0.05)和救治完成时间(t=2.349, P<0.05)的成绩高于对照组。结论TBL结合CBL教学方式提高甲状腺外科实习医师临床教学的效果,以及处置危机的团队协作能力,具有应用和推广价值。

Abstract: Objective To explore the application of team-based learning(TBL) combined with case-based learning (CBL) in training program of thyroid surgery on medical intern. Methods A prospective study was conducted in 94 interns with training program from November 2020 to June 2021 in thyroid surgery, Ruijin Hospital. The interns were grouped chronologically. Forty-six interns from November 2020 to February 2021 were included into control group and 48 interns from March 2021 to June 2021 into study group. Interns in control group were given traditional training and interns in study group with TBL with CBL training. Basic theory and knowledge, basic operative skill and ability of managing crisis were evaluated and compared between two groups. Results The basic theory and knowledge in study group were significantly better than that in control group (t=12.610, P<0.05). However, there was no significant difference in basic operative skill (t=1.566, P>0.05) and ability of managing crisis (t=1.313, P>0.05) between two groups. As for crisis management abi-lity, the team work ability (t=3.610, P<0.05) in study group was significantly better than that in control group and the completion time of patient rescue in study group (t=2.349, P<0.05) shorter than that in control group. Conclusions For learning thyroid surgery of medical intern, TBL combined with CBL training program could increase the effect of clinical teaching and team work ability of managing crisis, which would have prospects of application.
