摘要： 目的探讨选择性介入栓塞治疗(STAE)耳廓动静脉畸形(AVM)的疗效。方法回顾7年来我院治疗的耳廓血管畸形病例14例,比较采用单纯手术治疗、STAE结合手术治疗,以及单纯STAE治疗后的病变改善情况及复发率。结果单纯手术治疗,手术创伤大,失血量多,复发率较低;STAE后手术,手术时间缩短,出血大幅减少,手术野清晰,并发症减少,安全性提高;单独STEA治疗,短期内易复发,但通过选择更好的栓塞剂或技术,其效果将会大幅提高。结论选择性介入栓塞治疗在耳廓动静脉畸形的治疗中起着重要的作用,是一种重要的治疗手段。

Abstract: Objective To explore the effect of superselective transcatheter artery embolization (STAE) in the treatment of auricular arteriovenous malformations (AVM). Methods The clinical results in the treatment of auricular AVM of 14 cases during the past 7 years were reviewed. The different treatments including operation, STAE and combined therapy were compared. Results Operation resulted in severe trauma and large amount of bleeding but with low recurrent rate. While operation after STAE shorted operation time and extremely decreased the blood loss with clear operation field and less complications. The STAE might obtain better results with the advanced embolism drugs and modified technique, although the early recurrence was common recently. Conclusion STAE is playing more and more important role in the treatment of auricular AVM and should be recommended as an important treatment method.
