    Isolation and Culture-Expansion of Rat Compact Bone-Derived Mesenchymal Stem Cells

    Objective To introduce a detailed protocol for isolating and expanding mesenchymal stem cells(MSC) from Wistar rat compact bones.Methods Wistar\'s MSC were isolated and culture-expanded from bone marrow by Percoll density gradient centrifugation or from compact bone debris by digestion with collagenase typeⅡ.The morphological and proliferation features between the two populations of MSC were compared.The in vitro osteogenesis and adipogenesis were identified by intracellular alkaline phosphatase activity and droplets of lipids respectively.Results MSC were successfully isolated and culture-expanded by two different methods.Both of these two populations took fibroblast-like morphology and exhibited similar proliferative activity.They could be induced into adipocytes and osteoblasts in vitro under the defined conditions,which were revealed by positive Oil-Red O and alkaline phosphatase staining.Conclusion Both bone marrow and compact bones could be served as the sources for isolating rat MSC,and the biological properties of the harvested cells are comparable.

    Preliminary Study on Subcutaneous Tissue-engineered Cartilage with Cartilage Microparticle Acellular Matrix and Fibrin Glue as Scaffold

    Objective To evaluate the possibility of formation of subcutaneously implanted tissue-engineered cartilage using allograft chondrocytes mixed with xenogenic cartilage microparticle acellular matrix(CMACM) and fibrin glue(FG)as scaffold.Methods Chondrocytes were isolated from ears of adult New Zealand white rabbits.CMACM from porcine auricle was established and combined with FG and exoteric amplified chondrocytes.The compound was transplanted into the back of rabbits subcutaneously.The experiment was divided into three groups according to compound components。Group A,allograft chondrocytes mixed with CMACM and FG;Group B,autograft chondrocytes mixed with CMACM and FG;Group C with CMACM and FG.Five adult rabbits were used,and six regions were divided on the back of every rabbit.Each rabbit was injected compound of group A,B and C in two regions.Specimens were evaluated macroscopically,then harvested and detected histologically at 8 and 12 weeks after operation.Results In group A and C compounds could not form subcutaneous cartilage tissues.While cartilage tissues formed with group B mixtures at 8 weeks;At 12 weeks,new-formed cartilage tissues seemed even more stabilized without inflammatory around it.The size of new-formed cartilage was obviously shrinked with small vessels growing into it and the thickness of new-formed cartilages was only 1 mm.Conclusion The autogenic chondrocytes combining CMACM and FG matrix is a potential material to the cartilage tissue engineering.The volume and thickness of new-formed cartilage tissue is limited and micro-vessles growing into cartilage may contribute to longer survival for new tissues.

    Preliminary Study of Co-Transplantation of SVF-PLGA with Fat Complex on the Construction of Tissue Engineering Implant

    Objective To evaluate the feasibility and safety of co-transplantation of SVF-PLGA with fat complex on the construction of tissue engineered implant.Methods Forty adult New Zealand male rabbits were randomly divided into four groups.In the experimental group A(n=10),implant was constructed by use of SVF-PLGA with fat complex and in the group B (n=10).SVF-PLGA complex buried by adipose tissue was used.In the control group C(n=10) only PLGA and group D(n=10), SVF-PLGA complex was used.All the complex were translated between skin and superficial fascia.Tissue samples obtained were analyzed with gross observation,HE stain and red oil stain at 4 months postoperatively.Results The tissue engineered implant was mainly vascularized adipose tissue without tumor cell.PLGA scaffolds were degraded completely 4 months later. The survival rate of fat in experimental group B was higher than in the group A and in the control group B.Conclusion SVF-PLGA with fat complex on soft tissue with the advantage of high survival rate of fat.

    Thrombin-Activated Platelet-Rich Plasma Promotes the Proliferation of Human Marrow Mesenchymal Stem Cells

    Objective To investigate whether the supernatants of thrombin-activated platelet-rich plasma could be used as a substitute for the isolation and culture-expansion of human mesenchymal stem cells.Methods Human marrow mesenchymal stem cells(MSC) were cultured in media containing the supernatants of thrombin-activated platelet-rich plasma (tPRP) and fecal calf sera(FCS) selected from lots.Their proliferation status was evaluated by MTT and carboxyfluorescence diacetatesuccinimidyl ester-labeling assays.Furthermore,the phenotypic characteristics were analyzed by flow cytometry and the differentiations along adipogenic and osteogenic pathways were assessed by histological staining.Results MSC cultured in medium containing tPRP displayed a typical fibroblast-like morphology and,compared with(FCS),tPRP exhibited more potent activities to support the expansion of MSC.Flow cytometry showed that they were homogenously positive for CD29, CD73,CD166 and HLA-ABC and negative for CD31,CD34,CD45 and HLA-DR.Differentiation assays showed that of MSC cultured with tPRP occupied the capacities of in vitro osteogenesis and adipogenesis.Conclusion It is concluded that human tPRP might be an optional substitute to FCS for MSC proliferation.

    The Expression and Significance of Secreted Protein,Acidic and Rich in Cysteine in Hypertrophic Scar and Keloid Tissue

    Objective To investigate the expression and significance of Secreted protein,acidic and rich in cysteine (SPARC) in keloid and hypertrophic scar(HS).Methods Tissue samples were harvested from 26 patients with keloid and 26 with HS(3-12 months) and 5 from the skin of healthy adults.All specimens were fixed in 40 g/L formalin.Then the specimens were continuously sliced into 4μm thick sections.These sections were stained by streptoavidin-biotion method of immunohisto-chemistry.The other groups were normal control(normal skin),positive control(gastric cancerous tissue) and negative control(phosphate buffer) instead of The first antibody.The concentration of Rat anti-human SPARC monoclonal antibody were 1:200.Results There was scarce expression of SPARC in the skin tissue of healthy adults,and it was only present in the dermal papillae at the dermis epider-mis conjunctions and partly in the blood vessels and skin appendages adjacent to the basement membrane.There was low expression of SPARC in the dermal scar tissue and skin appendages in HS.Also,early expression of hypertrophic scar sputum.There was no significantly difference in the expression between late hypertrophic scar(P>0.05).SPARC showed scattered diffuse distribution as the same in the hypertrophic scar,and the lower expression of positive signals in the keloid.Compared with that in normal skin,there was no significant difference in the SPARC expression between HS and keloid(P>0.05).Conclusion There was lower SPARC expression in HS(3-12 months), and lower expression in keloid.

    The Application of VEGF and bFGF Via Low-Frequency Ultrasound-Mediated Transdermal Drug Transport System in the Improvement of Maximizing Survival of Fascio-Cutaneous flap with Super Length Ratio in Pigs

    Objective To explore the effects of single or combined application of VEGF and bFGF via the noninvasive medication of ultrasound-mediated transdermal drug transport(UMTDT) in the survival rate and microcirculation of fasciocutaneous flap with big ratio of length to width.Methods A total of 100 fascio-cutaneous flaps were performed on the sides of the back in ten pigs.The flap area ranged from 3 cm×15 cm.These flaps were randomly divided into 5 groups as follows. A:control group with blank ultrasound;B:VEGF by low-frequency UMTDT;C:bFGF by low-frequency UMTDT;D:VEGF and bFGF by localized injection in multiple points;E:VEGF and bFGF by low-frequency UMTDT.The quantitative analysis of the vitality of the flaps,capillary intensity and expression index were observed 10 days after operation.Results There was significant difference between the group of combined medication of both and bFGF via UMTDT and the other four groups in flap survival rate,capillary intensity and expression index.Conclusion The combined medication of VEGF and bFGF via UMTDT can significantly enhance the survival rate of fascio-cutaneous flaps with big ratio of length to width.

    Objective To explore the clinical effect of low abdomen island flap in the treatment of severe electric burn of scrotum.Methods From Oct.2001 to Dec.2009,8 patients with severe electric burn of scrotum were treated by the method of low abdomen island flap.The flap dimension ranged from 8 cm×11 cm to 10 cm×12 cm.Results The incisions at the donor site obtained healing by first intension.All 8 flaps survived completely.Conclusion The low abdomen island flap is effective in the treatment of severe electric burn of scrotum with its simple operation and low complication rate.

    Objective To explore the effects of local skin flap in combination with thread embedding in the repair of cleft lip with nasal basilar deformity.Methods A total of 36 cases were followed up in this study.The results were reviewed extensively,and a corresponding evaluation was made.Results All incisions obtained healing by first intension and the aesthetic appearance was satisfactory during follow-up period of 6 months to 2 years.Conclusion The above mentioned technique could be a reliable way to repair the cleft lip with nasal basilar deformity.

    Clinical Application of the Temporal Rhytidectomy with Suspension Technique

    Objective To estimate the clinical effect of the temporal rhytidectomy with suspension technique.Methods Twenty eight cases with low-middle-degree laxation of temple cutis were performed temporal rhytidectomy under local anaesthesia by the temple suspension.Results Fifteen patients were followed up for 1-2 years.The results were satisfactory with few complications.The aesthetic results were good.The laxation of ageing appearance can be apparently improved. Conclusion The temporal rhytidectomy combined with the suspension technique is a simple,reliable operation with less blood loss and without leave obvious traces from the surgery.

    Uniclip and K-wires for Trans-scaphoid Perilunate Fracture with Dislocation

    Objective To evaluate the role of uniclip and K-wires in the management of trans-scaphoid perilunate fracture with dislocation.Methods From February 2006 to September 2008,a retrospective study was done in 11 patients of trans-scaphoid perilunate fracture with dislocation.All patients were treated with uniclip and K-wires,aging 18 to 39 years.Scaphoid was fixed by an uniclip with pressure after dislocation were reduced;and a k-wire was placed from the capitate into the lunate to maintain reduction.Articular capsule and ligament were also repaired during the operation.Wrist joint began to activate at 8 weeks postoperatively following the k-wire was took away.The X-ray films showed that the scaphoid bony began to fuse and the uniclip was removed.All the 11 patients were followed up for 6-24 months.Results Curative effects were assessed by Wrist examination including wrist range of motion,grip strength,pain,X-ray views or computer tomography.The scaphoid fracture healed in all cases 4-10 months after operation.The range of movement of the wrist and grip strength was good.Conclusion Open reduction and internal fixation with uniclip and K-wires obtained satisfactory results in terms of anatomical restoration,the repair of articular capsule and ligament.It has superiority over other methods with more stability,direct viewing and less complications.

    Treatment of Intertrochanteric Fracture in Aged Patients by Different Methods

    Objective To discuss the clinical effect and indication of different methods for the treatment of intertrochanteric fracture in aged patients.Methods A total of 58 aged patients with intertrochanteric fracture were analyzed retrospectively.Anatomical locking plate of proximal femur was used in group A(n=14).Dynamic hip screws(DHS) and anatomical plate of proximal femur were used in group B(n=24) and group C(n=20) respectively.The bleeding volume, operative and healing time were compared with different methods.Results All patients were followed up for 8-36 months (18 months on average).According to Sander\'s for scoring,the choiceness rate was 92.9%,92%and 90%in group A,B and C.Conclusion In the treatment of intertrochanteric fracture in aged patients,different methods have own indication.The anatomical locking plate is effective for aged patients with severe osteoporosis.The DHS fixation is suitable for EvansⅠ,Ⅱand someⅢtype fracture.The anatomical plate of proximal femur was used in EvansⅣandⅤtype fracture.

    Three-Dimensional Computer Tomography Characteristics of Re-dislocation Following Reduction of Hip Joint Dislocation

    Objective To investigate and analyze the orthopaedic morphology of re-dislocation after reduction of hop joint dislocation by using 3-dimensional computer tomography.The causes of re-dislocation were analyzed.Methods Twenty-eight cases of developmental dislocation of hip joint with re-dislocation were analyzed by quantitative and qualitative parameter using 3-D CT,and compared with some normal ones.Results The results show that the angle between femoral neck and femoral diaphysis in re-dislocation group were larger than no re-dislocation group and normal one.The incidence of severe deficient of superior acetabulum in re-dislocation group was higher than group with successful operation.The anterior angle of femoral neck was significant difference between re-dislocation and normal group.There was no difference between group of re-dislocation and group with successful operation.Conclusions The causes of re-dislocation of hip joint reduction may be correlated with the angle between femoral neck and femoral diaphysis and the defect of superior acetabulum rim.

    Safety and Patients Satisfaction in Breast Cancer Patients with Immediate Reconstruction after Radical Mastectomy

    Objective To evaluate the therapeutic outcome and patient satisfaction in breast cancer patients with immediate reconstruaction after radical mastectomy.Methods Twenty two patients with breast cancer were treated by immediate reconstruction of latissimus dorsi musculo-cutaneous flap after radical mastectomy(group A).Twenty two patients with breast cancer were treated by radical mastectomy(group B).Results According to Harris for cosmetic effect of reconstructive breasts and according to postoperative satisfaction surveyor\'s rod for patient satisfaction.In group A,the results were classified as excellent in 14 cases,good in 5 cases and fair in 3 cases.There were no local recurrence and distal metastasis in both groups.There was significant difference in satisfaction scoring between group A and B(P0.05).Conclusion Radical mastectomy with immediate reconstruction of latissimus dorsi musculo-cutaneous flap has advantages of satisfactory results,cosmetic appearance and patient satisfaction.So it is an ideal and reliable choice for breast reconstruction.
