上海地区部分中老年糖尿病患者体内维生素水平分析

Analysis on levels of vitamins in middle-aged and elderly patients with diabetes mellitus in Shanghai

摘要: 目的:调查上海地区部分中老年糖尿病患者体内的维生素水平变化情况。方法:收集2016年1月1日至2016年11月30日本院门诊1 100名中老年体检者的资料,将其中264例糖尿病患者纳入糖尿病组,836例非糖尿病体检者纳入非糖尿病对照组。所有观察对象均检测了维生素(A、B1、B6、B12、叶酸、C、E)及糖化血红蛋白(hemoglobin A1c,HbA1c)。研究分析糖尿病组与对照组间的维生素水平的异同情况;并将糖尿病患者进一步以HbA1c≥7.0%为切点,分为血糖控制达标组(HbA1c<7.0%,192例)与未达标组(HbA1c≥7.0%,72例),进行亚组分析,观察HbA1c与其体内维生素水平间的相关性。结果:与非糖尿病组相比,糖尿病组的维生素B12水平偏低[(408.82±163.53) pg/mL 比(437.49±206.82) pg/mL],差异有统计学意义(P<0.05);而其余维生素(A、B1、B2、B6、叶酸、C、E)在2组间差异无统计学意义(P均>0.05)。以HbA1c≥7.0%为切点,糖尿病患者中,血糖控制达标组与未达标组间的维生素B12水平差异有统计学意义[(422.30±172.92 pg/mL 比(372.89±129.68) pg/mL, P<0.05)。线性相关分析显示,HbA1c与各种维生素水平间无统计学相关性(P值均>0.05)。结论:糖尿病患者体内的维生素B12水平较非糖尿病患者偏低,而且其中血糖控制欠佳者的维生素B12水平更低。额外补充维生素B12可能对糖代谢紊乱患者有益。

Abstract: Objective: To investigate the changes in vitamin levels in middle aged and elderly patients with diabetes mellitus in Shanghai. Methods: A total of 1 100 middle aged and elderly participants undergone health examination were enrolled during January 2016 to November 2016. Of these participants, 264 diabetic patients were categorized into the diabetic group and the other 836 non-diabetes were served as the control group. All the subjects were tested for vitamins (vitamin A, B1, B2, B6, B12, C, E and folic acid) and glycosylated hemoglobin (HbA1c), and differences in vitamin levels between diabetic group and control group were compared. According to the level of glycosylated hemoglobin (HbA1c), patients with type 2 diabetes mellitus were further divided into 2 groups: well-controlled group(HbA1c<7.0%) and poor-controlled group (HbA1c≥7.0%). The correlation of HbA1c with vitamins in the body was assessed. Results: Compared with controls, diabetes mellitus patients had a lower serum vitamin B12 level [(408.82±163.53) pg/mL vs (437.49±206.82) pg/mL P<0.05]. Patients with HbA1c≥7.0% had lower level of vitamin B12 than patients with HbA1c<7.0%[(372.89±129.68) pg/mL vs(422.30±172.92) pg/mL, P<0.05]. Linear correlation analysis showed that level of glycated hemoglobin had no correlation with serum levels of other vitamins (all P>0.05). Conclusions: Compared with non-diabetes, patients with diabetes have lower serum vitamin B12 level, and poor controlled diabetic patients have lower serum vitamin B12 level than well-controlled diabetic patients.
