    Navigation and Virtual Surgery Assisted Multidisciplinary Treatment of Multiple Maxillofacial Fractures

    Objective To discuss the effectiveness of multidisciplinary treatment for multiple maxillofacial fractures guided by surgical navigation and virtual surgery.Methods From January 2011 to December 2016,46 cases with multiple maxillofacial fractures were enrolled.The preoperative plan was developed through interactive collaborative discussion among the experts and the personalized virtual operation design on the surgical navigation software platform.Then the patients were treated with first-phase open reduction through the guidance of surgical navigation.By comparing the differences between 3D reconstruction of the postoperative CT scans and preoperative plan,the outcomes were evaluated.Results Fractured bone fragments were reduced to the planned positions and verified by probe-based navigation.Most patients healed without serious complications.The assessment of surgical discrepancy showed an optimal result that the error was less than 2 mm (1.56-±0.27 mm).Conclusion Multidisciplinary management with the assistances of virtual surgery and navigation technology,can provide more accurate and efficient treatment plan,and is a highly valuable choice for this complicated treatment procedure.

    Priliminary Exploration of Digital Evaluation in Craniofacial Neurofibroma Reduction

    Objective To investigate the application of three-dimensional (3D) laser scan technology combined with MRI and CT examination in the measurement of craniofacial neurofibroma's volume.Methods From May 2015 to September 2016,11 patients with craniofacial neurofibroma were scanned by 3-D laser,MRI and CT.Mimics 18.0 software was used to reconstruct 3-D model of tumor and face,analyze and calculate 3-D model volume.All patients underwent 3-D laser scan again to evaluate the effects of the operation 3 months after operation.Results ① Volume of planned resection part was (61.8±67.5) cm3 according to MRI pre-operation.Volume of resected tumor was (47.0±55.4) cm3 during operation,accounting for (79.0%±16.4%) of the MRI volume.② The volume of the surgical head (bounded by the median line) was reduced to (54.4±60.5) cm3 scanned by 3-D laser,of which the reduction rate was (9.4%±6.4%).The actual volume reduction rate was (9.3%±5.8%) in terms of estimated in operation.There is no statistical significance (P=0.977) between two calculate methods.③ Based on the 3-D laser scan technology,the symmetry of the preoperative area (bounded by the median line) was (72.7%±1.1%),while it turned out to become (79.9%±9.3%) after operation.The degree of symmetry increased (10.6%±8.1%) with statistical significance (P=0.000 4).Conclusion 3-D laser scan technology is an effective supplement to MRI and CT.It would probably become a low-cost,convenient,direct,objective and accurate method to measure the volume of craniofacial neurofibroma,which can be used in guiding the operation,assessing the results,and monitoring tumor variation after operation.

    Clinical Efficacy of Endoscopy Assisted Secondary Breast Augmentation

    Objective To explore the efficacy and complication of the secondary breast augmentation with assisted endoscopy.Methods From February 2012 to June 2015,endoscopy assisted breast augmentation was performed on 60 patients who needed revisionary breast augmentation.The patients who were not satisfied with the outcome,who suffered severe fibrous capsule contracture and the patents who suffered prosthesis fracture were included.All the patients were evaluated for efficacy and complications.The follow up period was 1 year.Results The procedure was performed successfully on 60 patients.No prosthesis rupture or prosthesis displacement was found.Except 3 patients who suffered from grade Ⅲ Baker?fibrous capsule contracture again,all the patients were satisfied with the results.Conclusion With assisted endoscopy,homeostasis,proper visualization,dissection of fibrous capsule and prosthesis space have been facilitated,at the same time,cosmetic benefits for patients have been maximized through limiting and camouflaging scars.In conclusion,endoscopy assisted breast augmentation is worth promoting.

    Changes of Masseter Muscle after Mandibular Angle Osteotomy

    Objective To investigate the changes of masseter muscle after mandibular angle osteotomy.Methods A total of 26 patients were treated with mandibular angle osteotomy from July 2009 to June 2015.Three-dimensional reconstruction by maxillofacial CT was used to measure the maximum cross-sectional area of masseter muscle before operation,10 days and 1 year after operation.Meanwhile,the masticatory efficiency was measured and the variance analysis was performed.Results The maximum cross-sectional area of masseter muscle increased 10 days after operation (P<0.05).Compared with the pre-operation,the maximum cross-sectional area decreased slightly 1 year after operation,and there was no significant difference between the 2 groups (P>0.05).The masticatory efficiency decreased significantly 10 days after operation (P<0.05).Compared with the 10 days post-operation,the masticatory efficiency increased significantly 1 year after operation (P<0.05).And there was no significant difference of masticatory efficiency between 1 year post-operation and preoperation (P>0.05).Conclusion The masseter muscle function can be restored 1 year after mandibular angle osteotomy.

    The Nursing Effica cy of Four Kinds of Cream in the CDT Treatment of the Lower Limb Lymphedema

    Objective To observe and compare the nursing effects of four skin care creams for the treatment of lymphedema in lower limb.Methods Sixty patients with secondary lower limb lymphedema were divided into four groups,① The VITAS soothing cream gel group (group 1,n=15),② The LR Cellona cream group (group 2,n=15),③ The lincomycin cream group (group 3,n=15);④ The lanolin cream group (group 4,n=15).Before manual lymph drainage,all indicators were observed and recorded.Patients in each group were treated with the appropriate cream.The standards of drainage and bandage operation were unified.All patients were given one course of CDT,observation indexes were recorded.The observation indexes before and after treatment were statistically analyzed.Results The average response strength of the cream was 1.93,1.47,3.27,and 2 respectively.The comfort level was 67.42%,71.65%,65.06% and 72.16% respectively.The average improvement rate of keratosis was 55.6%,33.3%,64.3% and 52.6% respectively.The average improvement rate of drying was 0%,100%,100%,100%;The average improvement rate of desquamation was 100%,62.5%,100%,87.5%.Conclusion Four kinds of skin care cream have different advantages.LR Cellona cream has the smallest skin reaction;Lanolin cream has the beat comfort;VITAS soothing cream gel has the fastest anti-itch effect;Lincomycin cream has the beat anti-keratosis effect;Lincomycin cream,lanolin cream,LR Cellona cream have the best anti-drying effect;VITAS soothing cream gel and lincomycin cream have the best anti-desquamation effect.

    Effectiveness of Picosecond 755 nm Alexandrite Laser for the Treatment of Melasma

    Objective To evaluate the clinical efficacy,safety and recurrence rate of picosecond 755 nm alexandrite laser in the treatment of melasma.Methods All 51 patients with melasma were treated with picosecond 755 nm alexandrite laser combined with a focus lens array for 5 treatment sessions every 3 weeks.The treatment parameter:0.4 J/cm2 of fluence,8 mm of spot size,5 Hz of frequency and 550×10-12 sec of pulse duration.Patients were all followed up for 8 weeks.Outcomes assessments including photography,melasma area and severity index (MASI) score and related adverse reactions were recorded.The factors affecting the treatment were analyzed.Results The mean MASI score decreased dramatically from (12.04±6.54) to (6.82±5.63) after 5 laser treatment sessions,the decreasing ration of the MASI score was about 47.22%.But after 8-week follow up,the mean MASI score slightly increased to (7.67±5.63).No obvious side effect was observed.Through the statistical analysis,disease duration,severity,wood's lights typing,Fiztpatrick typing,and family history of melasma had impact on the prognosis of melasma.Conclusion The novel picosecond 755 nm alexandrine laser is effective and safe with lower recurrence rate in the treatment of melasma,and is worthy of promotion.

    A Simple and Practical Method for the Treatment of Axillary Osmidrosis

    Objective To explore an effective method for treating axillary osmidrosis and decreasing complications after operation.Methods A 2-3 cm long skin incision was designed in the middle and lower of the axillary crease and the undermine scope of tissue was over 1 cm out of the axillary hair growth area.The sweat glands and subcutaneous tissue attached to the dermis were removed manually with scissors and curettage and the skin was repaired into full-thick skin flap.After operation,the skin was anchored to the subcutaneous tissues with more than 10 sutures using interrupted suturing technique.8-shaped compression dressing was wrapped to each axilla.Results All 208 patients were followed up for 6 to 12 months.Malodor elimination was totally treated in 200 of 208 (98%),fair in 8 (2%) and the efficient rate was 100%.Recurrence of axillary osmidrosis occurred in 3 patients and 2 cases were cured by re-operation.No subcutaneous hematoma,effusion,infection and skin necrosis happened after operation and the incision healed well.No obvious obstacle in activity was observed and all patients recovered well.Conclusion Shaving together with curettage for removing the apocrine and eccrine glands and interrupted suturing technique to anchor skin has the advantages of effective resection,high successful rate and fewer complications.

    Craniometaphyseal dysplasia (CMD) is a rare genetic bone disorder characterized by progressive hyperostosis and sclerosis of the craniofacial bones and abnormal metaphyseal widening of the tubular bones.CMD has two major modes of inheritance,autosomal dominant (AD) form (OMIM #123000) and autosomal recessive (AR) form (OMIM #218400).In this paper,a 7-year-old male with AD CMD was reported.The patient presented with a history of congenital craniofacial deformities and hearing loss,and was later confirmed by ANKH mutation.

    Report of one case of nevoid basal cell carcinoma syndrome.A 44 years old male patient with facial painless black rash for more than 1 year came to our department.Dermatological examination:maculas and papules on the face and around the ear,railiary to bean-size,black and brown,well-demarcated,some with roughness surface and uneven pigment distribution,slightly uplift.Visible scattered needle-sized retraction was seen in the palms and toes.Pathological examination showed basal cell carcinoma.Diagnosis:nevoid basal cell carcinoma syndrome.

    Craniosynostosis represents a kind of skull deformity caused by the premature fusion of one or more cranial sutures,which often changes the patient's appearance,sometimes obstructs the normal development.Although the exact pathogenesis of early cranial suture closure is still largely unknown,the fibroblast epidermal growth factor (FGF) seems to have affected the normal development of the cranial sutures.In this paper,the recent genetic researches related craniosynostosis and those identified pathogenic genes were summarized,mainly focuses on the FGF factors,to help the genetic diagnosis and the study of cranial sutures defects.

    Poland syndrome is a rare deformity of the chest wall accompanied with the abnormality of the trunk,limbs and other parts.As the incidence is low,the uniform etiology is not yet settled,but there are various theories of it.The multi-system and multi-site clinical manifestations make it more difficult to diagnose it overall,and the presence of some serious complications will also affect the timing of treatment and strategy choice.At present,the diagnosis and treatment of Poland syndrome is often done by thoracic surgery,hand surgery,plastic surgery separately and lack systematic classification,severity score system and treatment strategies.In this paper,the latest progress of the diagnosis and treatment of Poland syndrome were introduced in detail from the aspects of etiology,clinical manifestation and treatment strategy.At the same time,the future development of diagnosis and treatment for Poland syndrome was prospected.
