    Reconstruction of Tissue Engineered Skin Substitute Containing Appendages Using Dermal Papillae Cells And Keratinocytes

    Objective To reconstruct tissue-engineered skin substitutes containing appendages using human dermal papillae cells and keratinocytes together according to the developmental mechanism of the hair follicles.Methods Dermis substitutes of the experimental group were reconstructed by mixing both keratinocytes and dermal papillae cells into collagen gel.The collagen gel was seeded onto the surface of dermal substitutes.No cells were seeded onto the dermis in control group.The composite skin substitutes were cultured at the air-liquid interface for 5 days.A rounded full-thickness skin defect in diameter of 1cm was made on the back of the nude mice and then covered with the skin substitute.The healing and contraction of wound was observed.At 4,6,8 weeks after transplantion,samples were obtained.Results The wound healing time was 2 and 4 weeks in experimental and control group respectively.The wound contraction was more greater in control group than in experimental group.Hair follicles and sebaceous glands formed in the dermis of the experimental group at 6 weeks after transplantation.Conclusion In corporation of dermal papillae cells and keratinocytes into dermal substitutes is a promising method to reconstruct tissue-engineered skin substitutes containing appendage.

    Preliminary Study on Human Umbilical Cord Mesenchymal Stem Cells Transplantation in the Treatment of Late Traumatic Epilepsy

    Objective To explore the clinical efficacy of human umbilical cord mesenchymal stem cells transplantation in the treatment of late traumatic epilepsy.Methods Thirty-one patients with late traumatic epilepsy were transplanted with human umbilical cord mesenchymal stem cells and the clinical effect was observed.Results During 18.7 months of follow-up,21 patients were found to be completely seizures-free or with only aura(ClassⅠ),5 patients only had rare seizures(ClassⅡ,less than 3 times a year).And obvious improvement was achieved in 4 patients(ClassⅢ,at least 75% seizures reduction),no significant change was observed in 1 case(ClassⅣ,less than 75% seizures reduction).The satisfactory rate was 83.87%.Conclusion The human umbilical cord mesenchymal stem cells transplantation in the treatment of late traumatic epilepsy can result in a satisfactory clinical effect.

    Objective To investigate the risk factors of ventriculo-peritoneal shunt tube blockage after surgery in infant with hydrocephalus and the effect of preoperative administration of umbilical cord mesenchymal stem cells(Mesenchymal stme cells,MSC) on the incidence of this complication.Methods A retrospective analysis was conducted in 125 cases of infant with hydrocephalus to evaluate the occurrence of shunt blockage during the follow-up of 2 years after ventriculo-peritoneal drainage.Further,the incidence and its associated factors in 11 cases who received intravenous infusion of MSC treatment preoperatively were also analyzed.Results The clinical risk factors included preoperative cerebrospinal fluid cell count,cerebrospinal fluid protein content,age,terminal of location drainage tube and serum albumin concentrations orderly.MSC treatment significantly increased serum albumin level and decreased the occurrence of drainage tube obstruction(1/11 vs.53/114,P<0.01).Conclusion Preoperative infusion of MSC can increase the level of serum albumin and reduce post-operative complications.

    Effects of Combination Chemotherapy with Cisplatin and 5-fluorouracil on Abdominal Flap in Dogs

    Objective To investigate the effects of preoperative chemotherapy with cisplatin and 5-fluorouracil on abdominal flap in dogs.Methods Twenty-one male mongrel dogs were randomly divided into 3 groups(n=7): chemotherapy group,control group and blank control group.The dogs were treated with cisplatin(DDP,50 mg/m2) and 5-fluorouracil(5-Fu,150 mg/m2) in chemotherapy group.Chemotherapeutic drugs were intravenously injected on the 1,3 5 day.Each cycle went for 21 days,a total of 2 cycles.The dogs only received an injection of same amount of saline and glucose in control group,while nothing in the blank control group.The flap surgery was performed 42-49 days after injection and the changes of the flaps were observed after operation.On the 30th day after operation,the flap survival area was measured by means of coordinate transformations,then the entire flap was removed for pathological analysis.Results A total of 17 dogs survived(5 in chemotherapy group,6 in control group and 6 in blank control group).In the 2-3 days post operation,different degrees of swelling was observed in each flap.All flaps survived 30 days after operation.The survival area was similar among chemotherapy group,control group and blank control group(P=0.910).The epidermis and subcutaneous tissue grew well observed by light microscope.No obvious thrombosis and inflammatory cells were found in the inferior epigastric artery and veins in dogs of three groups.Conclusion Two cycles of preoperative chemotherapy with cisplatin and 5-fluorouracil has no significant influence on the survival area of abdominal flap.

    Effect of Vacuum-Assisted Infection Control and Granulation Formation on Soft Tissue Explosive Defects

    Objective To observe the effect of early NPWT application on infection control and granulation formation in explosive soft tissue defects by comparing the bacteria colony-forming unit and pathological changing.Methods Explosive soft tissue defects were created in pig model.Twelve defects were created on 3 pigs and divided into 2 groups randomly.NPWT group was on one side and Gauze group on opposed side.Each group had 6 wounds.Exposure group was composed of another 2 pigs with 6 wounds.At 6 hours,Day 3,Day 5 and Day 7,specimens were harvested in the center of the wound for counting of bacteria and HE staining.Microscopy was used for observation Granulation formation was observed by microscope.Results The numbers of bacteria colony on NPWT group were smaller than that of Gauze group and Exposure group at Day 3,5 and 7.The counting of NPWT group peaked at Day 3 and declined below the critical value of infection at Day 5.After the NPWT application,the wound area shrank and became superficial.Compared with Gauze group,more granulation formation,and obvious staining with fibroblast and endothelium cell,as well as edema fading of the cells were observed in NPWT group.Conclusion Compared with the Gauze group,early application of NPWT can effectively control the bacteria multiplication and promote granulation formation on soft tissue explosive defects.

    The Quantitative Research and Clinical Application of Facial Symmetry

    Objective To explore the facial symmetry and morphological feature of general population so as to provide criteria for facial reconstruction and cosmetic surgeries.Methods Fifty young women were selected randomly without any facial deformity.The method of photogrammetry was adopted to measure the facial symmetrical distances and angles.The measuring results were compared between the left and right face.Then,single-half face composite portraits were created by computer for 10 cases,which were compared with the original images.The composite portraits and original images were scored respectively.SAS software was used to analysis the results of the two experiments.Results The included angle between the horizontal line and the line connected inner and outer canthus,the included angle between the horizontal line and the line connected the superciliary head and peak,the distance from Al to Ch,the distance from Ch to Go and the distance from Go to Meg showed statistically significant difference between the left and right half face.Scores of composite portraits were significantly higher than that of original images.Conclusion The asymmetry in middle and lower face is more obvious than that in upper face.Absolute facial symmetry does not exist.The concept of symmetry should not be applied independently and simply in the aesthetics of human body.The standardization study of morphological character on facial symmetrical is still in progress.

    Biomechanical Study of Rigid Internal Fixation for Mandibular Angle Fracture Through 3-D FEM Simulation

    Objective To observe the stress distribution of mandibular angle fracture under different rigid internal fixation(RIF) methods by developing a three-dimensional finite-element method(3-D FEM).Methods CT scan technology and related software were used to develop a 3-D FEM of mandibular angle fracture under different RIF.On this basis,the mandibular stress distributions were analyzed by MSC.Marc software.Results The von Mises stress mainly concentrated on the middle of the titanium plate in tension band fixation,while in two mini-plate fixation,it concentrated on the middle of the superior titanium plate.In the tension band fixation,the max-values of displacement was more than that in the two mini-plate fixation.When the biting force dropped to 102.7 N(LMOL) and 40.3 N(ICP),the furthest fracture mobility got into margin of safety under the tension band fixation.Conclusion The two mini-plate fixation was more stable than the tension band fixation.Choosing the suitable occlusion,tension band fixation could also provide sufficient stability for the mandibular angle fracture.

    Copper Needle with Low Voltage Direct Current in the Treatment of Disseminated Cavernous Hemangioma

    Objective To explore the application of copper wire with continuous low voltage direct current in the treatment of disseminated cavernous hemangioma.Methods Twenty-three patients with disseminated cavernous hemangioma were implanted with copper wire that electrified with continuous low voltage direct current for 10 minutes and the copper wire was detained for one week.Results Among the 23 patients,15 patients were cured,5 patients were mainly cured,others got effective results.Conclusion Copper wire with low voltage direct current is an effective,simple and low-injury mothod for the treatment of the disseminated cavernous hemangioma and should be advocated in primary hospitals.

    Nerve Graft with Two End-to-Side Neurorrhaphy in Treating Bilateral Chronic Proper Digital Nerve Injuries

    Objective To introduce a new method for the repair of the bilateral chronic proper digital nerve injuries.Methods From November 2000 to October 2009,40 cases with bilateral chronic proper digital nerve injuries were divided into 3 groups.Fifteen cases in Group A were conducted nerve graft with two end-to-side neurorrhaphy.Thirteen cases in Group B were conducted nerve graft with one end-to-side nerve neurorrhaphy.Twelve cases in Group C were performed nerve transplantation with end-to-end nerve anastomosis.Results After 6-8 months of follow-up,the anaesthesia and pain of the injured fingers disappeared completely and finger pulp was plump with good flexibility in group A.The measurement of the sense was S4-S4+ and 2-PD was 5.0-6.0 mm.The feeling recovery was not synchronization and the operative side felt more obvious in group B.The measurement of the sense was S1-S3 and 2-PD was 7.0~7.6 mm.In Group C,the measurement of the sense was S3-S3+,SI or even blind on the finger pulp,and 2-PD was 7.5~8.5 mm.Conclusion Nerve transplantation with end-to-side nerve anastomosis were a new valuable method for the repair of bilateral chronic proper digital nerve injuries.

    The Clinical Experience in Treatment of Traumatic Foreign Bodies on Hands

    Objective To investigate some neglected problems in treatment of traumatic foreign bodies on hands.Methods Foreign bodies were removed on 126 cases which were checked by X-ray before and after operation in recent ten years.Some were checked by ultrasound before operation.All the patients were followed for 1 to 8 months.Results There were 65 cases with foreign bodies under nail.Among them,18 cases had rough nails after nail extraction and others had good shaped nails after partial resection of nail.Foreign body with foreign body-fish bones was remained after extraction in 1 case.One needle foreign body remaining over 2 years and getting rusty was fractured in operation.In the 15 cases punctured by cactus,5 cases got swelling after operation.One of two explosive injury cases remained some foreign bodies after operation and caused infection.Conclusion Cases with foreign bodies under nail should avoid nail extraction in case of crude nail bed and deformation nail.Metallic foreign bodies with long persistence should be removed with surrounding soft tissue in case of fracture of metallic foreign body.A wide debridement is necessary when the hand was punctured by cactus and a thorough debridement is needed after explosive injury by gunpowder.
