    Functional Testing and Evaluation Tactics of Artificial Cervical Disc Prosthesis In Vitro

    Artificial cervical disc replacement which retains the movement of adjacent vertebral segments, reduce the stress level of adjacent segments, is conducive to preventing the degeneration of adjacent segments. At present, the domestic application of artificial cervical disc prosthesis is mainly imported products, most of domestic similar products are in the process of R&D and production. The in vitro evaluation methods of prosthetic cervical intervertebral disc prosthesis in vitro was presented, including subsidence, creep, expulsion, subluxation testing, dynamic and static compression, shear testing, wear testing. Combined with the biomechanical data of human cervical spine, the clinical safety and effectiveness of the product was evaluated. Based on the open data of the FDA on artificial cervical disc prosthesis, the results of in vitro functional tests of 5 different designs of artificial cervical disc prosthesis were analyzed and compared. The acceptable criterion for the in vitro evaluation test of artificial disc prosthesis was discussed,which can provide a reference for the R&D of domestic products.

    Differences in the Clinical Phenotypes of Apert Syndrome with the FGFR2 Mutation Site Ser252Trp and Pro253Arg: Based on Meta Analysis

    Objective To explore the differences in the phenotypic effects of two common mutations (Ser252Trp and Pro253Arg) in patients with Apert syndrome. Methods The key terms "Apert and FGFR2", "Apert syndrome and FGFR2 mutation" were retrieved in PubMed and CNKI databases respectively, and 29 literatures containing gene mutations and clinical phenotypes were selected from 227 articles. A total of 230 cases were reported, involving 37 clinical manifestations. The male to female ratio was 1:1, and the average age was 8.9±9.6 years. The correlation between clinical phenotypes and gene mutations were compared by t test, chi square test or Fisher's exact test. Results The two common mutations were detected in 87% of our patients with Apert syndrome, lower than the previously reported 98% level. The frequency of cleft palate in Ser252Trp was 2.3 times as high as that in Pro253Arg (55% vs 24%, P<0.001), and the frequency of type Ⅲsyndactyly(hands)in Pro253Arg was significantly higher than that in Ser252Trp(69% vs 29%,P<0.001),while there was no significant difference in other clinical manifestations between the two mutations. Conclusion According to the relevant literatures during 1995-2017, the frequency of two common mutations in Apert syndrome may be overestimated. There are subtle differences in the clinical phenotypes between two mutations in the Apert syndrome. Cleft palate is significantly more common in patients with the Ser252Trp mutation. In contrast, the syndactyly for hands is more severe in patients with the Pro253Arg mutation.

    Effectiveness Comparison Between Pulsed Dye Laser and Long-Pulsed Nd:YAG Laser in the Treatment of Fresh Scars

    Objective To evaluate and compare the effectiveness of pulsed dye laser(PDL)and the long-pulsed Nd:YAG laser in the treatment of fresh scars. Methods Patients with fresh scars were involved for a comparative randomized split-scar trial. Each scar was divided at the midline into two fields. One half received treatment by pulsed dye laser (Vbeam, Candela Corp.) with a 7 mm spot size at 0.45 ms with 6~9 J/cm2and a 20 ms spray duration with a 20 ms delay. The other half was treated by Nd:YAG (GentleYAG, Candela Corp.) with a 3 mm spot size at 10 ms with 150~180 J/cm2. Patients received 3 laser treatment sessions at one month interval. The scars were evaluated with Vancouver scar scale (VSS) 2 months after treatment. Results Twenty-one patients with average 3.4-week scars were involved. Two months after last treatment, final total VSS analysis revealed significant decline of both PDL and long-pulsed Nd:YAG laser group after treatment (P<0.05), but no statistical difference between the 2 groups (P>0.05). However, the emphasis was different. The PDL sites advantaged in the improvement of vascularity and thickness of scar, whereas Nd:YAG in pliability. Conclusion Both 2 kinds of lasers could improve the appearance of fresh scars but emphasize differently.

    Imaging and Pathological Characteristics of Port-Wine Stain with Facial Hypertrophy

    Objective To analyze the imaging and histopathologic characteristics of PWS patients with facial hyperplasia, in order to improve the therapeutic effect of the disease. Methods Thirty-two patients with facial hyperplasia treated by laser was retrospected. The degree of hyperplasia and the density of enhanced signals on T1-weighted MRI were graded using quartile scale. The pathological data of 18 cases was retrospectively analyzed and compared with 15 PWS patients without facial hyperplasia, to explore the histological difference. Results MRI results showed statistically significant correlation between the degree of hypertrophy and the density of enhanced signals (P<0.001). Histological features showed that vascular malformation existed not only in the dermis but also in the subcutaneous fat and muscle with much larger vessel diameters(0.048 mm vs 0.020 mm,P=0.004;0.035 mm vs 0.017 mm,P=0.008)and thicker vessel walls(0.014 mm vs 0.006 mm, P=0.003;0.010 mm vs 0.006 mm,P=0.020). Conclusion The involvement of the subcutaneous tissue by CM, which exceeds the damage depth of laser for blood vessels,might be one of the major cause of poor therapeutic effect with laser therapy.

    A Retrospective Analysis of the Factors Affecting Implant Retention Rate after Mandibular Reconstruction

    Objective To explore the factors affecting implant retention rate after mandibular reconstruction with fibula bone flap and provide theoretical direction for clinical therapy. Methods From January 2015 to December 2017, a total of 20 cases (male 12cases, female 8 cases,20 to 45 years old, average 27.2 years old) who received dental implantation after mandibular reconstruction were retrospectively analyzed. The data such as the status of keratinized gingiva, the thickness of peri-implant soft tissue and the height of bone graft were collected, and were all compared with the marginal bone loss (MBL). Results Of all the 20 patients, keratinized gingival loss was observed in 12 cases bilaterally, 5 cases unilaterally, and preserved in 3 cases. Three months, 6 months and 12 months after the reconstruction, the thickness of soft tissue were (5.88±0.54)mm,(3.46±0.30)mm,(3.17±0.31)mm,separately;and the height of bone graft were(2.36±0.23)mm,(2.17±0.19)mm, (2.05±0.22)mm,separately.Three months,6 months and 12 months after the implantation,the mesial MBL were(1.09±0.32)mm, (1.22±0.37) mm, (1.36±0.46) mm, separately;and the distal MBL were (1.10±0.29) mm, (1.25±0.31) mm, (1.35±0.39) mm, separately.Statistical analysis revealed that the keratinized gingiva and thickness of soft tissue were both positively correlated with the MBL,and the height of bone graft was inversely correlated with the MBL.Conclusion For the oral implant patients after mandibular reconstruction,the height of the bone graft should be increased and the thickness of soft tissue should be reduced.And keratinized gingiva should be preserved as much as possible if oncological safety is allowed during mandibular resection.

    Effects of Two Kinds of Operation on the Incidence of Postoperative Fistula and Speech Intelligibility in Older Children with Cleft Palate

    Objective To explore the effects of two kinds of operation (levator veli palatini retropositioning & levator veli palatini retropositioning combined with buccal fat pad flap and pharyngeal flap) on the incidence of fistula and speech intelligibility in older children with cleft palate. Methods Sixty cases with cleft palate(4 to 9 years old)were divided into 2 groups randomly, and were treated with levator veli palatini retropositioning (group A) and levator veli palatini retropositioning combined with buccal fat pad flap and pharyngeal flap(Group B)respectively.The incidence of palatal fistula and speech intelligibility in the 2 groups were observed and compared. Results The incidence of fistula in group A was higher than in group B (P<0.05). The speech intelligibility was improved in both group A and group B (P<0.05). The speech intelligibility and scar formation in group B was both better than in group A (P<0.05).Conclusion For older children with cleft palate, levator veli palatini retropositioning combined with buccal fat pad flap and pharyngeal flap can reduce the incidence of palatal fistula effectively, and also can obtain better speech intelligibility. Since this method can avoid bone exposure, so as to reduce scar formation and the effect on maxillary growth, it is a functional cleft palate repair method and worthy to be recommended.

    Comparison of Speech Intelligibility between von Langenbeck Palatoplasty and Double-Opposing Z-Plasty Repair in Children with Cleft Palate

    Objective To explore a best surgical method for the correction of velopharyngeal closure by comparing the speech intelligibility between von Langenbeck palatoplasty and double-opposing Z-plasty in children with cleft palate. Methods From 2009 to 2013, 69 cases with cleft soft palate were selected. Thirty-five cases were repaired using double-opposing Z-plasty (double-opposing Z-plasty group), others were repaired using von Langenbeck palatoplasty (von Langen-beck group).All cases were followed up after the age of 3.5 years.A single blind hearing was conducted by 3 phonetics,and the speech intelligibility between the two groups was compared. Results The speech assessment showed that the average speech intelligibility was 88.72%±6.05% in double-opposing Z-plasty group, which was significantly higher than that in von Langenbeck group 71.31%±3.46%. Conclusion Double-opposing Z-plasty repair can decrease the pharyngeal cavity and increase the length of soft palate to reconstruct the soft palate muscle,which is beneficial for the correction of velopharyngeal closure.

    Evaluation of the Curative Effect of Intramedullary Proximal Humeral Nailing in the Treatment of Elderly Proximal Humeral Fracture

    Objective To explore the feasibility and the curative effect of proximal humeral nailing in the treatment of elderly proximal humeral fracture. Methods From January 2015 to December 2016, 49 cases above 70 years old with elderly proximal humeral fracture (NeerⅡ-Ⅳ)were included and reviewed.All cases were divided into 2 groups according to different internal fixation and treated with locking proximal humeral plate (LPHP group) or intramedullary proximal humeral nailing fixation (IPHN group) respectively. The comparability of the basic data of two groups, recovery of shoulder joint function and imaging data were analyzed. Results The difference was considered statistically significant between two groups in the assessment during the early postoperative period(P<0.05).There was no significant difference in the assessment of long-term function after operation(P>0.05).And there was no significant difference in the assessment of cure rate between the two groups (P>0.05). Conclusion IPHN may promote the early recovery of elderly proximal humeral fracture, but its long-term effects have no obvious advantage.

    Stem cell therapy holds great promise in tissue regeneration and reconstruction. However, the underlying mechanisms remain elusive due to the lack of effective and noninvasive approaches during therapeutic processes. Within different strategies of molecular imaging, lanthanide-based up-conversion nanoparticles (UCNP) have been explored as imaging agents for luminescence imaging, magnetic resonance imaging (MRI) and computerized tomography (CT). As luminophore, UCNPs are preferable to other bioimaging probes. In this paper, the development history, preparation, the research status of stem cell tracer and the implications of tumor related research of UCNP were reviewed.

    Vascularized composite allograft transplantation is an important method in tissue repair and reconstruction. However, the side effects of immunosuppressive therapy remains the great burden of these patients and lacks an appropriate solution. There is a growing attention to the emerging cell therapy, like mesenchymal stem cell, especially adipose derived mesenchymal stem cell,in combine with immunosuppressive agents which may prolong the graft survival and reduce the side effects of immunosuppressor. The underlying mechanism of adipose derived mesenchymal stem cell therapy may include the paracrine of immunomodulatory molecules, the induction of regulatory T cells, mixed chimerism and the reduction of ischemia reperfusion injury. Moreover, the safety, timing and dosage of adipose derived stem cell should be tested and determined before clinical application.

    Perforator flap has enjoyed a wide popularity in tissue repair, however, its application on reconstruction of massive superficial defects has been impeded by the small diameter of perforator artery and the limited volume of usable flap. After an extensive review of literatures published recently,this article delineated an up-to-date progress in basic and clinical research on expansion of perforator flap vascular supply, including the perspective of vascular anatomy and flow characteristics of perforator flap as well as relevant therapeutic strategies to increase flap blood supply and promote tissue vascularization.

    An important factor affecting the anterior teeth esthetics is getting the accurate three-dimensional position, besides selecting a good planting time and patients themselves.In computer-assisted implantation surgery,to reduce surgical risk and improve aesthetic effects,the digital template utilizes 3D techniques to simulate prostheses.Therefore,it can get the right direction and depth of the drilling. In this way it can avoid damaging important anatomical structures, so that it guarantees the accurate position. However, there are still problems with low accuracy. In this paper, the research status of digital template implantation in anterior teeth esthetics was reviewed.

    Application and Progress of Ulthera in the Treatment of Skin Laxity and Rhytides

    Microfocused ultrasound is a new technique to treat skin laxity and rhytides. Over the past several years, the safety and efficacy of microfocused ultrasound have been well documented and widely used in brows,periorbital region,lower face, neck, chest, buttocks, arms, thighs and knees. As a novel and non-invasive technology, it has the following characteristics: Firstly, it can precisely and selectively focus the thermal injury at pre-selected depths without heat injury;Secondly, the "see and treat" system allows visualization and treatment simultaneously; Thirdly, ultrasound energy is not absorbed by melanin, thus it is safe even for darker skin types theoretically. In this paper, the progress of ulthera in the treatment of skin laxity and rhytides was reviewed from the aspects such as pre-procedure preparation, procedural approach, complications and outcomes.
