    Experimental Study of Constructing Tissue Engineered Bone with Canine Bone Marrow Mononuclear Cells in Vitro

    Objective To explore a method for instantly constructed TEB to meet the instantly clinical requirements of repairing bone defects. Methods BMNCs of Beagle were separated by 1.063 g/mL Percoll gradient solution. The ideal seeding density of 30 ×106 cells/mL was obtained for canine bone marrow mononuclear cells (cBMNCs) on partially demineralized bone matrix (pDBM). The cell activity and osteogenic capacity of immediately constructed TEB composed of cBMNCs and pDBM were analyzed in vitro. Results Certain cell activity and osteogenic capacity were confirmed in immediately constructed TEB under the osteogenically inducted environment in vitro. Conclusion The directly replantation of instantly constructed TEB might satisfy the immediately clinical requirements of repairing bone defects.

    Objective To establish the technological route of ultrasound and CT image fusion based on optical tracking system and to apply in soft tissue navigation of craniomaxillofacial surgery. Methods (1) To acquire two-dimensional ultrasound images and three-dimensional reconstruction through the nearest neighboring difference algorithm;(2) Ultrasound and CT image fusion through the dLDP algorithm independently researched; (3) Clinial verification and evaluation of the effect of image fusion. Results The independently researched image fusion algorithm based on dLDP realized the fusion of craniomaxillofacial ultrasound and CT, the average error was about 1.96 mm. Conclusion The accuracy and display effect based on the dLDP algorithm can meet the clinical requirements. It has important significance in independent research of craniomaxillofacial soft tissue navigation system.

    The Eff ect of Facial Nerve Crush Injury and Ischemia on the Recruitment of Macrophages in Rats

    Objective To study the influence of the facial nerve crimping ischemia injury on the recruitment of macrophage in rats, and to provide theoretical basis for clinical application of traumatic facial paralysis. Methods Sixty SD rats were randomly divided into 3 groups: 20 in ischemia (extrusion+ischemia) group; 20 in extrusion (extrusion only) group;20 in control group. Rats in ischemia group received an operation that extruding the left facial nerve, cutting off the surface nutrition blood vessels of the facial nerve and stripping epineurium under operating microscope (ischemia completely); The rats in extrusion group just received the extrusion on the left facial nerve but the nutrition blood vessels were preserved; The control group rats served as sham group. At 3 days, 7 days,14 days after operation, behavioral observation and electrophysiological detection were given to each group. After assessment, 5 rats were sacrificed in every group for immunohistochemistry (using macrophage specific antibody CD68), in order to study macrophages further. Results 1. Behavioral observation: At 3 days after operation, all rats in ischemia group and extrusion group showed facial paralysis. Then, at 7 days after operation, part of the rats' facial paralysis were aggravated, the other rats had recovery performance; At 14 days after operation, facial paralysis recovery was observed in experimental groups. However, the recovery rate of the ischemia group is significantly slower. 2. Electrophysiological detection: With the extension of time, orbicularisoris muscles compound action potential maximum amplitude and latency period in ischemia group and extrusion group were gradually approached to the control group. But didn’t achieve the normal level. The recovery rate in ischemia group was relatively lower than in extrusion group. 3. Histological observation: Immunohistochemical results showed that the macrophages gradually increased in ischemia group, until reached a peak at 7 days after operation; The result in extrusion group was similar but the number of macrophages was more than in ischemic group; No significant positive expression was observed in control group. Conclusion Facial nerve crush injury and ischemia can hinder the recruitment of macrophages, eventually hinder the process of nerve repair.

    Key Gene Screening of Skin Regeneration Under Mechanical Stretch Stimulation in Rats

    Objective To determine the expression changes of skin regeneration-related key genes under mechanical stretch stimulation in rats and to screen key genes. Methods Skin expansion model was established and skin samples were harvested: the unexpanded control group (con), water-injected 60 mL group (exp60), water-injected 120 mL group (exp120) and water-injected 180 mL group (exp180). Immunohistochemical staining was implemented in the sliced the sample, stain-ing proliferation marker Ki67. According to that, the trend curve of Ki67 positive rate of samples was drawn. Gene Chip was also undertaken to reveal the gene expression changes during expansion process. T test and fold change of differential ex-pression were applied to screen key genes. Then PCR was used to confirm the relationship between the expression of the key gene and the proliferation of skin. Results Immunofluorescence staining showed that, the expression of the proliferation marker Ki67 descending order is con, exp180, exp60, exp120. This study screened 11 skin regeneration-related key genes of rats under different mechanical stretch stimulation: Myh1, Myh2, Myh3, Myh4, Car6, Pi15, Apt12a, Mt1a, Cthrc1, Rsad2, IGF-BP5. Compared to the expression of control group, IGF-BP5 was a down-regulated gene. PCR showed that the expres-sion of IGF-BP5 descending order was: exp120, exp60, exp180, con. Conclusion Under mechanical stretch stimulation, with the increasing time of expansion and amount of water injected, skin regeneration of rats shows accelerated proliferation at early stage, active proliferation in the mid-term and tendency to slow down the proliferation in the later period; IGF-BP5 may be a key regeneration-related gene of rats under mechanical stretch stimulation.

    Effects of 17β-estradiol on the Synthesis of Collagen and Elastin in Cultured Human Skin Fibroblasts

    Objective To explore the influence of 17 beta estradiol (17β-E2) with different concentrations on the synthesis of collagen and elastin of cultured human skin fibroblasts (hSFB) at different time points in vitro. Methods Human fibroblasts were cultured with different concentrations of 17β-E2 (10-7, 10-8, 10-9, 10-10, 10-11 mol/L) for 24 hours, 48 hours and 72 hours. The corresponding RNA was extracted at respectively time points, then mRNA expression of type Ⅰprocollagen, typeⅢprocollagen and tropoelastin were detected by reverse transcription-polymerase chain reaction (RT-PCR) method. Results The mRNA expressions of procollagen Ⅰ, procollagen Ⅲ and tropoelastin were not changed in 24 hours, but up-regulated in 48 hours and declined in 72 hours by 17β-E2 stimulations. On 48 hours, the synthesis of procollagen and tropoelastin stimulated with 10-10 mol/L of 17β-E2 was significantly increased, compared with the other concentrations. On 48 hours with the same concentration, the up-regulated effect on synthesis of procollagen and tropoelastin was different, the effect on procollagen was stronger than tropoelastin, especially procollagen Ⅲ. Conclusion 17-E2 has a certain role in promoting the synthesis of collagen and elastin, though the influence is different according to different culture time and different concentration, and the most effective concentration and culture time are 10-10 mol/L and 48 hours respectively.

    The Safety Assessment of an L-shaped Polyethylene Sheet Implant for Nasal Elongation

    Objective To evaluate the safety of L-shaped polyethylene sheet implant for nasal elongation. Methods From January 2007 to December 2009, 78 patients received nose elongation were concluded for retrospective study. 38 patients received L-shaped polyethylene sheet implantation were involved in observation group, while 40 patients without polyethylene sheet implantation were involved in control group. All occurring complications were recorded. The incidence of complications was analyzed by Fisher's test. Results Patients were followed up for average 30.6 months (18 months to 5 years). In observation group, the incidence of complications was 7.8 %, and one major complication was observed during the follow-up period. In control group, the incidence of complications was 12.5% without major complication. The exact probability showed no significant difference between the observation and control group. Conclusion The high density polyethylene is a kind of safe and effective implant material for nasal elongation.

    Clinical Application of Segmental Excision Technique in Facial Scar Plasty

    Objective To observe the effect of segmental excision technique in the treatment of facial scar. Methods From January 2012 to June 2014, 38 cases with facial scar were treated. Based on the skin tension lines, facial organ zonation and contour lines, the whole scar was segmented into two or more segments. Then segmental scars were excised and repaired by plastic technique. After 3-6 months, the punctuate scars were treated with fractional laser skin regeneration technique or directly excision. Results All cases were followed up for 1 to 3 years. The color and texture of scar were improved, moreover, no apparent contracture, incision lines were obscured. Facial contours were restored without distorting surrounding structures. Conclusion Segmental excision technique combining secondary fractional laser skin regeneration technique for the punctuate scars is an effective method for facial scar treatment, especially irregular linear scar and contracture scar.

    Risk Factor Analysis of Joint Dysfunction in Limb Venous Malformation

    Objective To investigate the determined risk factors of joint dysfunction in patients with limb venous malformations. Methods Patients with venous malformation of limbs were collected from Jan 2013 to Aug 2015. Their initial presenting symptoms were categorized into the dysfunction and control groups. Independent risk factors for dysfunction were identified between the two groups. Results A total of 168 patients with venous malformation of the limbs were identified. Among them, 44 (26%) patients presented as flexion or extension limitation and varus or valgus were enrolled into the dysfunction group. In univariate analysis, age, location, size, involved tissue, visual analogue scale of pain and pain duration were significantly associated with joint dysfunction (Pone year) remained independently associated with joint dysfunction. Conclusion It highlights that the importance of an early and precise diagnosis in limb VMs. Pain relief is also a key approach to prevent joint dysfunction and pain should be under control in early.

    Clinical Features and Treatment Strategies of Secondary Cleft Lip Resulting from Ulcerated Hemangiomas

    Objective To explore the etiology, clinical features and treatment strategies of secondary cleft lip resulting from ulcerated hemangiomas. Methods A retrospective analysis of IH from Dec 2012 to Jan 2014 was performed. The etiology, clinical features and treatment strategies of secondary cleft lip resulting from ulcerated hemangiomas were analyzed. Results The medical records of 528 IH were examined. 33 IH (20 girls and 13 boys) located in lip area. Only 3 developed ulceration, and 2 of 3 resulted in secondary complete cleft lip, both girls. Propranolol was used at 2 mg/kg/d to treat the IH just one week after ulceration, the cleft lip inevitably happened after medicine therapy. Both patients received surgical repair of cleft lip and obtained good results. Conclusion Cleft lip is a rare complication of ulcerated hemangioma. Propranolol can not prevent cleft lip after the ulceration of IH absolutely. An early stage repair surgery at the beginning of involuting phase is more helpful in reducing pain and uncomfortable of both physical and psychological than the repair after an absolute involution.

    Objective To explore the differential diagnosis of NF1 and vascular malformation using genetic testing. Methods A 10-year old boy’s right calf, with scattered erythema, has enlarged without definite boundaries for one year. More than 6 Café au lait, with diameter larger than 1.5 cm, scattered all over the trunk and limbs. CT: capillaries and superficial vein of right calf increased and enlarged, with an impression of tendency to venous malformations. In accordance with NF1 diagnosis criterion, it could not be diagnosed as NF1 without other typical symptom except caf é au lait. Then NF1 gene detection was performed. Results NF1 nonsense mutations was detected after DNA sequencing, c.C. 4006 C>T, p.Gln 1336 * (hybrid), supporting for NF1 diagnosis. Conclusion Genetic testing can be performed to help the diagnosis for the patients with atypical clinical manifestations.

    Café-au-lait macules (CALMs) are a benign epidermal hyperpigmentation disorder with an incidence of 10%-20% in the normal population. Histopathologic examinations have shown increased epidermal melanin without melanocytic proliferation. Lasers are the most effective methods to treat CALMs. The efficacies vary and are influenced by multiple factors. Response rates range from 33%to 96%. Adverse effects may turn up after laser treatment. High recurrence rate is the obstacle that can't ignore and the recurrence rates range from 5%to 50%. The mechanism is still unknown.

    Intravascular administration of absolute ethanol is the most effective treatment for vascular malformation. However, catastrophic cardiovascular collapse, although rare but fatal, may occurred during the procedure. At present, the clinical characteristics and pathogenesis of this fatal complication are still controversial. Vital aspects which may involved in the pathogenesis: pulmonary hypertension, pulmonary embolism, cardiac depression were reviewed. And the clinical characteristics of the reported cases, focusing on the timing, the dosage of ethanol, prediction and prevention were also reviewed systematically.

    The pathological scar can cause serious psychological and physiological morbidity to the patients. How to treat the pathological scar is a challenge faced by plastic surgeons. With the advances in laser technology, as well as the accumulation of clinical experiences, laser technique has become an important method of scar treatment. It is capable of revising the pathological scar by destroying blood vessels and stimulating collagen remodeling within the scar. In this paper, the application of laser techniques for the treatment of pathological scar was reviewed.

    Nasolabial fold as a dividing line located between the zygomatic cheek and upper lip is one of the earliest signs of aging, which gradually deepened with the age. The treatment for nasolabial fold wrinkle is still a problem, which is difficult to conquer. People are more increasingly focused on how to apply the ideal technology to achieve the curative effect as long as possible and to reduce the recurrence as far as possible. In this paper, according to the latest literature available on the subject, the research progress on the treatment for nasolabial fold rejuvenation was reviewed.
