    Experimental Study on the Effect of Extracts of Akermanite on Proliferation and Osteo-Differentiation of Human Adipose-Derived Stem Cells

    Objective To elucidate the underlying mechanism of the osteo-differentiation of akermanite on human adipose-derived stem cells(hADSCs) via observing the effect of different diluted rate extracts on the proliferation and osteo-differentiation of hDASCs.Methods The original extract was prepared according to the ISO/EN 10993-5 and hDASCs were cultured in the different diluted rate extracts(1/2,1/4,1/8,1/16,1/32).The MTT assay was carried out to quantitatively evaluate the proliferation of the cells.Under the condition of osteo-induction,osteogenic differentiation of hADSCs cultured in the different extracts was detected by ALP expression and calcium deposition.Results The 1/2,1/4,1/8 extracts could concentration-dependently suppress the proliferation of hASCs,while the 1/4,1/8,1/16 extracts could promote the osteogenic differentiation of hADSCs.Among them,the 1/4 extract with the concentration of Ca2+,Mg2+ and Si4+ were 2.36 mM,1.11 mM,1.03 mM,has the optimum one.Conclusion The ions in the extract of akermanite can suppress the proliferation of hADSCs while promoting the osteogenic differentiation of hADSCs in a suitable concentration.

    Study on Biological Characteristics of Adipose Derived Stem Cells Transfected with HGF Gene by Adenovirus Vector

    Objective To investigate the effect of HGF transfection by adenovirus vector(Ad-HGF) on biological characteristics of human adipose derived stem cells(hADSCs).Methods hADSCs was transfected by Ad-HGF in MOI=200.The expression of HGF gene was detected by ELISA.The differences of biological characteristics such as cell morphology,growth curve,surface markers,cell cycle,differentiation potential between hADSCs transfected by HGF group and hADSCs group were observed.Results The expression of HGF was detected in supernatant 24 hours post transfection.The peak of it presented at 48 hours post transfection and sustained till 1 week after transfection.The hADSCs transfected by Ad-HGF grows as fibroblast-like style.There were no morphology and cell viability changes observed between transfected group and control group.Both two groups were shown to be positive for the expression of CD44,CD90,CD49d and CD105.On the other hand,expression of CD45,CD34,CD31,CD106 and STRO-1 were negative.There was no significant difference between two groups in cell cycle,adipogenic and osteogenic differentiation potential were observed.Conclusion The biological characteristics of hADSCs are not affected by adenovirus transfection.Adenovirus transfection is an ideal way to integrate HGF gene to hADSCs.

    Objective To explore the osteogenesis ability of purified hADSCs through flow cytometry sorting in vitro.Methods hADSCs were isolated and separated through flow cytometry using CD105 as surface marker.These cells were cultured in osteogenic-induced medium,CD105-hADSCs and non-seperated hADSCs were served as control groups.The cells were examined for alkaline phosphatase(AKP) and osteocalcin(OCN) by real-time PCR and western-blot after 2 weeks culture.Results The percentage of CD105+ hADSCs in monocytes was about 50% through flow cytometry sorting.Real-time PCR and western-blot both revealed the AKP and OCN activity of CD105+ hADSCs were significantly higher than CD105-hADSCs and non-seperated hADSCs after 2 weeks osteogenic-induction(P<0.05).Conclusion The hADSCs purified have good osteogenesis ability and could be the seeding cells for bone engineering.

    Objective To investigate the influence of hypoxia and co-culture of endothelial cells(EC) induced from BMSC and autologous bone marrow stromal cells(BMSC) on the proliferation of BMSC.Methods The rabbit BMSC were isolated and were induced into endothelial cells.These cells were divided into four groups.① BMSC group cultured under normoxia;② BMSC group cultured under hypoxia;③ BMSC+EC group co-cultured under normoxia;④ BMSC+EC group co-cultured under hypoxia.Inverted microscope and MTT method were applied to observe the effects of hypoxia and induced autologous EC on the cellular compatibility of BMSC.Results Endothelial cells could be induced from BMSC.Inverted microscope showed a cellular compatible growing status of BMSC and EC.The ability of proliferation of groups cultured under hypoxia were stronger than groups cultured under normoxia(P<0.01),but there was no significant difference between BMSC groups and BMSC+EC groups(P>0.05).Conclusion The hypoxia can increase the proliferation of BMSC.While the endothelial cells induced from BMSC can not.

    Experimental Study on Chitosan-Gelatin/β-Tricalcium Complex as Novel Scaffolds for Cartilage Tissue Engineering

    Objective To investigate the ultra-microstructure,physical characteristics and biocompatibility of the man-made porous chitosan(CS)-gelatin(Gel)/β-tricalcium(β-TCP) complex,so as to explore the feasibility of the complex as cartilage tissue engineering scaffolds.Methods The CS-Gel/β-TCP sponges were prepared by freezing and lyophilizing taking water as the hole-making agent.The ultra-microstructure was observed with scanning electron microscope.The compress strength and modulus were determined by omnipotent material-testing machine.The complex was subcutaneously imbedded in the rabbit to observe the biodegradability and biocompatibility.Results The solid content of the mixed solution,the affix quantity of β-TCP and the preliminary freeze temperature play the decisive role of the hole structure of the scaffold complex.The compress strength and modulus of the scaffold improved significantly with the affix quantity of β-TCP increased.And the mechanical function reduced when preliminary freeze temperature lowered.Immunological rejection was observed on early stage after embedded.Then the scaffold degraded,and was absorbed at 12 weeks.Conclusion The CS-Gel/β-TCP complex scaffold has a 3-D network structure with high porosity,good mechanical function and good biocompatibility.It may be regarded as one of the important cartilage tissue engineering scaffolds with latent value.

    Isolation of Specific Sequence of TGF-β Receptor Ⅱ Using a Phage 12-mer Display Peptide Library

    Objective To isolate the specific sequence of TGF-β receptor Ⅱ(TβRⅡ) from a phage 12-mer display peptide library and to evaluate their effects on keloid fibroblasts.Methods A phage 12-mer display peptide library was screened for four rounds using monoclonal anti-human TGF-β1 as the target.ELISA was performed to select monoclonal phages with good association for DNA sequencing.MTT assay was used to detect the effects of monoclonal phages on keloid fibroblasts.Results Five specific sequences were obtained,which were similar to TβRⅡ.The results of MTT assay showed that three of the five phage model peptides could inhibit proliferation of keloid fibroblasts.Conclusion Phage model peptides with specific sequences of TβRⅡ can inhibit proliferation of keloid fibroblasts.

    An Animal Model of Simple Blast Injury on Abdominal Wall with Visceral Exposure on Rabbit

    Objective To make an animal model of the simple abdominal wall blast injury with visceral exposure and to observe its long-term changes.Methods Twelve white rabbits of 8-10 weeks were randomly divided into two groups,and injured parts were marked under general anesthesia.A group:self-made enterocoelia protective device with 0.125 g TNT equivalent explosives ball on the site of injury;B Group:the same explosion source of injury without protective device.After injury,autopsy and pathologic examination were used to evaluate the animal model.Results Both groups of animal injury parts presented whole layer blast injury of abdominal wall was observed in both groups.The wound area of A group was(3.9+0.56) cm2,and B group was(3.83±0.49) cm2.There were no significant differences between the two groups(P>0.05).The survival time of A group was(141±87.78) h and B group was(3.67±2.25) h.There were significant differences between group A and Group B(P<0.05).The survival time of experimental group was longer in group A than group B.Autopsy and pathologic examination showed that the reasons for the early death in the group B was visceral injury and abdominal bleeding,while besides fiber adhesion,there was no obvious trauma on visceral in group A.Conclusion This experiment simulated the mechanism of abdominal wall blast injury.An animal model of simple abdominal wall blast injury with protective device can be used for long-term observation with effective protection.

    Reconstruction of the Blood Supply on Double Pedicle Deep Inferior Epigastric Perforator (DIEP) Flap

    Objective To introduce three different intra-flap anastomosis in reconstruction of the blood supply on double pedicle deep inferior epigastric perforator(DIEP) flap and to compare the outcomes of the three methods.Methods Eighty seven patients who received breast reconstruction by double pedicle DIEP flap in our department from January 2004 to December 2008 were reviewed.Three different intra-flap anastomosis in reconstruction the blood supply on double pedicle DIEP flap were performed.Thirty eight patients were performed the distal end to end anastomosis,20 patients were performed the lateral branch style and 29 patients were performed the end-to-side style.The operation time,postoperative vascular complications,flap loss,partial flap loss and fat necrosis of the three styles were analyzed.Results The length and width of the flap of the three groups had no statistical differences and so did the operation time.The follow up period ranged from 6 months to 60 months.2 patients of the distal end to end anastomosis,1 patient of the lateral branch style group and 1 patient of the end-to-side style group occurred postoperative vascular complications.There was no flap loss all of the three groups.One flap of the lateral branch style group developed partial flap loss.3 patients of the distal end to end anastomosis group,4 patients of the lateral branch style group and 4 patients of the end-to-side style group were found fat necrosis three months after operation.Conclusion The three styles of intra-flap anastomosis are all good methods for reconstruction of blood supply on double pedicle DIEP flap and can be used in all situations.The three methods have no differences in operation time,postoperative vascular complications and the capability of the blood supply of double pedicle DIEP flap reconstruction.

    The Factors Influencing Post-Mastectomy Patients’ Attitudes Towards Breast Reconstruction

    Objective To explore the related factors influencing post-mastectomy patients’ attitudes towards breast reconstruction.Methods In PKU Third Hospital,PKU First Hospital,PKU People’s Hospital,and Beijing Cancer Hospital,post-mastectomy inpatients suffered from breast cancer were investigated with self-developed questionnaire.Descriptive,univariate,and multivariate Logistic regression analyses were used.Results One hundred and eight cases were counted finally,39.8%(43/108) of the patients thought the breast reconstruction reasonable.There was significant discrepancies in the factors of age,occupation,education,household income,child status,and whether the surgeons informed mammaplasty to patients or not.In binary Logistic regression analysis(forward:LR),only age,education,and whether to be informed of the option of reconstruction were the related factors.Conclusion More and more patients consider reconstruction as a reasonable option.Surgeons should update the knowledge of breast reconstruction,popularize mammaplasty for breast cancer patients and improve their attitudes towards breast reconstruction.

    The Application of Segmental Osteotomy in the Correction of Posttraumatic Orbital Fracture

    Objective To explore the feasibility and outcome of segmental osteotomy in the treatment of the eyeball dystopia resulted from orbitalzygomatic fracture.Methods Among 26 cases of unilateral non-comminuted orbitozygomatic fractures,segmental osteotomy technique was used to depart the displaced orbitozygomatic complex.The fragment with contribution to lateral inferior orbital structure was mobilized upward and fixed.On the contrary,the fragment without contribution to the orbital structure was not mobilized.Results The volume of fractured orbit and the globe projection were adjusted considerably.Subjective assessment of the patients’ globe position displayed that 23 cases were satisfied with the outcome and 3 cases thought it acceptable.After 1 year follow-up,clinic appearances of ocular malposition and orbital deformity have been improved and retrieved efficiently.Conclusion Compared to the traditional methods,the segmental osteotomy as a simple technique with less dissection can reconstruct the orbital anatomic structure and restore globe position effectively.

    The Application of Back Skin Grafts for the Repair of Large Skin Defects

    Objective To explore the advantages of back skin grafts in the repairmen of large skin defects.Methods Fifty patients with different causes of skin defects,including deep burn of functional parts,later scar contracture,huge tumor resection are in this study.According to the location and the size of defects,split thickness skin graft was harvested on the back and transfered to the defects.Results After operation,the patients were followed-up for 1-2 years,all graft skin survived completely,and the function and appearance of the transplant area were good,with no scar hyperplasia or little.Conclusion Taking back as the donor site of skin graft is an ideal choice for the repair of large skin defects.

    The Application of Orbital Periosteum Fixation in the Treatment of the Upper Eyelid Laxity

    Objective To investigate the effects of orbital periosteum fixation in the treatment of upper eyelid laxity.Methods According to the traditional method,the surgical incision below the eyebrow was chosen then some sagging skin was removed.In addition.orbital periosteum fixation was added into the procedure in order to maintain the clinical effect in the treatment of upper eyelid laxity.One hundred and eight patients were performed this operation between 2000 and 2005.Results All patients were followed up for 1-5 years and were satisfactory with the results.Conclusion Orbital periosteum fixation is easy to operate.The traditional method plus orbital periosteum fixation can achieve long-term effect.

    Minimally Invasive Surgical Treatment of Clavicle Fracture with Closed Reduction and Cannulated Compression Screw Fixation

    Objective To explore the outcome of the treatment of clavicle fracture with closed reduction and cannulated compression screw fixation.Methods Fourty two patients with clavicle fractures were treated with closed reduction and cannulated compression screw fixation from January 2006 to January 2010.Results All patients were followed up for 6-18 months.The union of fracture were all good in X-ray.There were no abnormalities,delayed healing and complications in all patients.The excellent rate was 97.6%.Conclusion This operation method is a kind of simple,effective and minimal invasive surgical procedure for the treatment of clavicle fracture.

    Objective To explore the efficiency of the open reduction and internal fixation for the treatment of calcaneal fractures.Methods Twenty four patients of calcaneal fractures on 27 feet(Sanders Ⅱ type:8 feet,Ⅲ type:19 feet) were all done the open reduction and fixation with bone grafting of autogenous iliac.Results The patients were followed up for 6-20 months,mean 13.4 months.There was no delayed union or nonunion,and 95% of the patients(Sanders Ⅱ type:97%,Sanders Ⅲ type:93%) got good results according to Maryland for foot scoring.Conclusion Open reduction and fixation with bone grafting is an effective method for the treatment of calcaneal fractures.

    Objective To observe the effect of mini-invasive curettage in the treatment of osmidrosis under tumescent anesthesia.Methods Eighty-two patients with osmidrosis were treated by using minimal invasive curettage under tumescent anesthesia.Results All cases were followed up for 6-12 months and gained satisfactory results without any axillary malodor.Conclusion The minimal invasive curettage under tumescent anesthesia is a simple and effective method with few complications in the treatment of osmidrosis
