    New knowledge of prophylaxis and treatment about cancer-associated thrombosis

    Cancer-associated thrombosis (CAT) is the second leading cause of death in cancer patients, second only to the progression of the tumor. It leads to extended hospitalization and re-hospitalization, resulting in a serious social medical burden. Tumor-specific gene mutations such as JAK2, ALK, and KRAS mutations are closely related to the risk of CAT in patients with solid tumors. Patients treated with immune checkpoint inhibitors have a high risk of CAT, with a 1-year cumulative incidence rate of 10.86%. Therefore, accurate risk assessment is important. The Khorana risk score (KRS) is the first model to assess CAT risk and has been validated in several studies. The new generation models are based on the improvement of KRS, but they have not yet undergone large-scale external verification, such as the Vienna CAT model, PROTECHT model, and CONKO model. Due to the strong correlation between tumors and CAT, several CAT-related prevention guidelines have been established domestically and abroad. All of these guidelines recommend that patients undergo preventive anticoagulation therapy after tumor surgery. Low molecular weight heparin (LMWH) is the most widely used anticoagulant drug to prevent CAT. New oral anticoagulants (NOAC) are also used to prevent CAT, despite the incidence of hemorrhaging increases, the overall benefits to patients outweigh the risks. The strategy for treating CAT is complex. For patients with non-gastrointestinal tumors, NOAC can be used as a first-line drug, while LMWH is recommended for anticoagulation in patients with gastrointestinal tumors. Since CAT patients receive anticoagulation and anti-tumor therapy at the same time, drug interactions need to be considered to avoid drug-induced bleeding, especially the impact of combined medication on NOAC pharmacokinetics. The ideal length of anticoagulation therapy for CAT remains unclear. Guidelines recommend that patients with CAT should indefinitely continue the anticoagulation therapy after completing 3 to 6 months of initial anticoagulation therapy. Therefore, when determining the course of anticoagulation therapy for cancer patients, physicians should evaluate the risk of recurrence of thrombosis and bleeding, and optimize the treatment plan.

    Clinical characteristics and risk factor analysis of 118 patients with cerebral venous sinus thrombosis

    Objective: To analyze the clinical characteristics and thrombotic risk factors of cerebral venous sinus thrombosis (CVST) in Chinese population, in order to provide reference for the clinical diagnosis, prevention and treatment of CVST. Methods: A total of 118 CVST patients who visited the Thrombosis and Hemostasis Clinic of Ruijin Hospital from January in 2015 to December in 2022 were enrolled. The clinical data, imaging characteristics, and thrombotic gene panel were collected, and independent risk factors for thrombosis, recurrence, and multi-site thrombosis in CVST patients were studied through univariate and multivariate logistic regression. Results: Among 118 CVST patients, 88.1% (104/118) patients firstly developed CVST at age younger than 45 years old.It revealed that 57.6% patients (68/118) only suffered CVST once,and 33.1% patients (39/118) experienced thrombotic events at least twice, while 37.3% patients (44/118) suffered multi-site venous thromboembolism (VTE). The superior sagittal sinus was the most common venous sinus to form thrombosis (77/118), and multi-sites of venous sinus were involved simultaneously in 54.2% patients (64/118). The risk factor screening for thrombophilia showed that 70.3% patients (83/118) carried at least one thrombotic risk factor, including 53.3% patients (63/118) with inherited risk factors and 32.2% patients (38/118) with acquired risk factors. The main risk factors for males were anticoagulant protein deficiency (35/66) and antiphospholipid syndrome (5/66), while the risk factors for females mainly included anticoagulant protein deficiency (20/52), pregnancy/postpartum period (11/47), and contraceptive use (5/47). Among 35 patients with unknown risk factors, more than 30% of them experienced recurrent or multi-site thrombosis, and the underlying etiology in these patients still need to be further investigated. Univariate and multivariate logistic regression analysis showed that both inherited risk factors (odds ratio, OR=21.643, 95% confidence interval, CI 9.455-49.544, P<0.0001) and acquired risk factors (OR=10.836, 95% CI 4.306-27.270, P<0.0001) were independent risk factors for the occurrence of CVST. Genetic risk factor (OR=2.270, 95% CI 1.021-5.048, P=0.044) was an independent risk factor for recurrent thrombosis in CVST patients. Conclusions: In Chinese population, CVST is more common in middle-aged and young populations.,with 70.3% patients carrying at least one thrombotic risk factor. Genetic risk factors are the main cause of CVST in China and the detectable rate is 53.3%, which is significantly higher than that (22%) reported abroad. The patients with genetic risk factors have a significantly increased risk of developing CVST or recurrent thrombosis. Identification of thrombotic risk factors, prevention, diagnosis and treatment should be carried out earlier for individuals with high-risk CVST.

    “Japanese Gastric Cancer Treatment Guidelines” have been important guidelines for the treatment of gastric cancer in Japan and worldwide. The English version “Japanese Gastric Cancer Treatment Guidelines 2021 (6th Edition)” published in January 2023 consists of two parts. The first part includes diagnostic and treatment methods, such as surgery, endoscopic resection, chemotherapy, follow-up. The second part addresses clinical issues, including recommendations and explanations for clinical hot topics. This article will provide a Chinese interpretation of the guidelines and briefly introduce our hospital's clinical practice in the diagnosis and treatment of gastric cancer.

    In 2022, the World Stroke Organization released two new reports regarding global stroke statistics in the International Journal of Stroke. The reports updated the global incidence and mortality of stroke, evaluated the effects of gender and geographic factors, and provided updated statistics on attributable risk factors associated with stroke. This article briefly interprets the two reports in the context of stroke prevalence and disease burden in China. 2019 Global Burden of Disease Study findings show that stroke remains the second-leading cause of death and the third-leading cause of death and disability combined in the world. From 1990 to 2019, the burden (in terms of the absolute number of cases) increased substantially, with the bulk of the global stroke burden residing in lower-income and lower-middle-income countries. Moreover, people under 70-year-old were observed to have significant increases in stroke prevalence and incidence. The five major risks for stroke globally include high systolic blood pressure, high body mass index, high fasting glucose, environmental particulate matter pollution, and smoking. Stroke prevention and treatment in China are facing great challenges. Stroke has become the leading cause of death and disability among Chinese adults, as well as the leading cause of disability adjusted life year lost. The incidence, prevalence and mortality of stroke in China are geographically high in the north, low in the south and prominent in the central part of the country; the prevalence is higher in rural areas than that in urban areas; the morbidity and mortality rates are higher in men than those in women; the average age of onset is lower than in developed countries; the overall disease burden of ischemic stroke is on the rise, but that of hemorrhagic stroke is on the decline. The awareness of stroke prevention and treatment was low. In conclusion, it is of strategic importance to actively establish a graded stroke prevention and control system that meets the national conditions.

    The passivity-based control (PBC) based on energy function has been studied for grid-connected converters to achieve a better performance. However, traditional PBC method relies on the accurate mathematical model of grid-connected inverter. In previous studies on PBC, the effect of digital control delay is rarely considered and the stability under grid impedance uncertainties is not discussed, especially in the capacitive grid or complex weak grid. To address these issues, this paper proposes an improved PBC method to reshape the output admittance for LCL-filtered grid-connected inverters. The system passive region is expanded up to the Nyquist frequency by adding a capacitor current feedback loop which can achieve active damping control of LCL resonant frequency under the wide range of grid impedance changes. The parameter design method is also presented for the proposed PBC control. To verify the correctness of the theoretical analysis, both simulation and experiments are conducted on a 3 kW grid-connected inverter prototype.

    High penetrated renewable energy has brought great challenges to the flexibility of the power system due to its volatility and intermittency. To improve the capacity of renewable energy accommodation, the flexibility reformation of thermal power units, the construction of gas-fired units, and the electrical energy storage installation are considered as effective solutions. Thus, an optimization model for power system planning scheme considering multi-type flexible resources with their different output characteristics is established. The simulation results on a modified IEEE 24-bus power system and 12-node natural gas system demonstrate the effectiveness of the proposed model. In addition, the applicability of different flexible resource planning schemes is comprehensively evaluated from the perspectives of economy, accommodation capacity, and carbon reduction, so as to meet the different planning goals.

    China is now witnessing an accelerated aging process,and active reaction is an important issue that we are facing. Osteoporosis and the resulting fractures are particularly noteworthy. In order to further standardize the diagnosis and treatment of primary osteoporosis, the Osteoporosis and Bone Mineral Disease Branch of the Chinese Medical Association has developed and released the “Guidelines for the Diagnosis and Treatment of Primary Osteoporosis (2022 Version)” based on the epidemiological characteristics and pathogenesis of the Chinese population, which combined high-quality medical research evidence from both domestic and foreign sources. Based on evidence-based medicine evidence, and clinical practice in China, this article will focus on the interpretation of two key clinical issues in the new version of the guidelines, namely “fracture risk stratification and treatment selection” and “treatment course of anti-osteoporosis drugs”. Regarding the risk of fractures, this guideline divides it into high and extremely high fracture risks, cancels the classification of low and medium risks, and for one of the extremely high fracture risk indicators “recent brittle fractures”,defines “recent” as “24 months”.Given that the FRAX model underestimates the fracture risk of Chinese patients, it is proposed to develop a highly operational fracture risk stratification criteria in clinical practice. Regarding drug treatment, patients who use bisphosphonates should be fully evaluated before entering the medication holiday, and regular follow-up is still required. Although the Food and Drug Administration (FDA) of the United States has lifted the warning of osteosarcoma and 24- month treatment limit for terlipide, the user manual of the drug in China still suggests a treatment period of 24 months. This guideline is based on high-quality evidence-based medical evidence both domestically and internationally, and may provide reference and research directions for clinical practice.

    In recent years, offshore wind turbines show the trend of large-scale development, the installation area of which has been expanding to the deep and far-reaching ocean. However, due to the harsh environmental conditions in the far-reaching ocean region, the traditional rotor-lifting method is facing many limitations. In contrast, the single blade installation technology has significant advantages in installation efficiency and safety, and has gradually become a new research hotspot. Based on the characteristics and difficulties of the offshore single blade installation technology, this paper investigates and summarizes the lifting equipment and key technologies involved in single blade installation section, including blade yokes, the single blade installation dynamic simulation model, and the active control technology. Among them, the research and development of novel single blade installation equipment and methods with active control technology are essential for large-scale offshore wind turbine installation in the far-reaching ocean region. Additionally, based on the development trend and prospect of offshore blade installation and the docking technology, it introduces some technical ideas, including single blade yoke with dynamic positioning function, and double hoop blade vertical installation auxiliary device, which are expected to solve the installation problem of large-scale offshore wind turbines.

    With the development of floating offshore wind turbine technology, engineering projects of large scale floating offshore wind turbine has begun to emerge. Because manufacturers keep the parameters of wind turbine confidential, there are many challenges in numerical simulation analysis of floating offshore wind turbines in practical engineering projects. In this paper, the key technologies on numerical simulation of floating wind turbine in the practical engineering project was studied. Through the equivalent thrust model, a numerical model of real floating wind turbine was established and the dynamics responses under a typical load case was analyzed. Based on the research results of this study, the difficulties in establishing the numerical model of floating wind turbine in practical engineering projects can be well solved. This is of great significance to promote the technical development and accelerate the commercialization of China's floating wind turbine industry.

    Development and prospects of ERCP in China

    Endoscopic retrograde cholangio-pancreatography (ERCP) is an interventional endoscopic technique aimed at the diagnoses and treatment of biliary and pancreatic diseases under the guidance of endoscopy and X-ray ima-ging. ERCP was introduced into China from the 1970s. In the 1990s, ERCP was gradually popularized and applied in China with the development of medical technique and improvement of equipment, more medical institutions started to carry out ERCP. After 2000, ERCP technology and equipment were further upgraded and improved in China. Many secondary and tertiary hospitals began to establish the ERCP centers. Meanwhile, workshops and training also facilitated the teaching and practice of ERCP and cultivated more professionals of ERCP. In recent years, the technical level of ERCP in China has been continuously improved, and technologies and equipments are constantly updated and iterated. Currently, ERCP has become one of important techniques in the endoscopic field, playing an important role in the diagnosis and treatment of biliary and pancreatic diseases. The vigorous development of ERCP in China is attributed to the advancement of medical science, the improvement of technology and the efforts of generations of ERCP specialists.

    The installation of offshore wind turbine monopile foundation requires a variety of ship machinery and lifting equipment, etc. If the suitabe selection of those above matters could make the completion of the construction objectives be achieved. In order to select suitable ship machinery lifting equipment, using project as example the compare between domestic and foreign monopile installation, the selection consideration of the main floating crane ship and the drivability of the hydraulic hammer are specifically analyzed through the verification analysis of the monopile stabilizing platform and the description of other additional installation points, which shows that the main considerations for the selection of marine machinery and lifting equipment for offshore wind turbine monopile installation in China and the verification analysis methods are useful. These can provide serve as for the selection of marine floating machine for monopile installation, for the design and verification of monopile stabilizing platform, and for the preparation of construction organization and scheme.

    Cardiac magnetic resonance image (MRI) segmentation has the features such as there is a lot of noise, the target areas are indistinguishable from the background, and the shape of the right ventricle is irregular. Although convolution operations are good at extracting local features, the U-shaped convolutional neural networks (CNN) structure hardly models long-distance dependency between pixels and can not achieve ideal segmentation results on cardiac MRI. To solve these problems, UConvTrans is proposed with a dual-flow U-shaped network by global and local information integration. First, the network applies the CNN branch to extract local features and capture global representations by Transformer branch, which retains local detailed features and suppresses the interference of noise and background features in cardiac MRI. Next, the bidirectional fusion module is proposed to fuse the features extracted by CNN and the Transformer with each other, enhancing the feature expression capability and improving the segmentation accuracy of the right ventricle. Besides, the parameters of network can be set flexibly because the transformer structure in the proposed method does not require pre-trained weights. The proposed method also strikes a better balance between precision and efficiency, which is evaluated on the MICCAI 2017 ACDC dataset. The results show that the network outperforms U-Net by 1.13% average dice coefficient while the parameter amount and the floating point operations are only 10% and 8% of the U-Net. Finally, the proposed method achieves a dice coefficient of 92.42% for the right ventricle, 91.64% for the myocardium, and 95.06% for the left ventricle respectively and wins the first place in the myocardium and left ventricle on test set.

    Marine renewable energy is abundant but difficult to store. The generation of green hydrogen from offshore renewable energy can provide a new idea for the storage of green energy. This paper mainly analyzes the key technologies of hydrogen production from offshore wind power, focusing on the four aspects of offshore water electrolysis hydrogen production technology, offshore hydrogen energy storage and transportation technology, offshore hydrogen production and storage equipment design, as well as offshore hydrogen production and storage scheme. This paper compares and summarizes the key technologies of offshore wind power hydrogen production in China, and puts forward relevant countermeasures and suggestions on this basis. It provides reference for the innovative development of offshore wind power hydrogen production technology and equipment in China.

    Driven by the carbon peaking and carbon neutrality goals, the power system is transforming to the new structure which is dominated by renewable energy and is facing a new supply-demand balance situation. Pumped storage, as the most mature energy storage technology at present, can provide flexible resources with different time scales to ensure the safety of the power system and promote the consumption of renewable energy. However, the operation strategy and cost sharing mechanism of the pumped storage station (PSS) are not clear, which hinders its further development under the new situation. In this context, the technical characteristics and functions of PSS are sorted out first. Then, the investment cost model is established from the perspective of the whole life cycle. After that, the evolution path of pricing mechanism and cost sharing mode are described in view of the different stages of electricity market development, providing a feasible scheme for the marketization of PSS. Finally, the future development of PSS is summarized and prospected.

    The Hull Structure Design and Practice of the Condensate Storage Tank of Semi-Submersible Platform

    There is little industrial experience and projects for semi-submersible platform with large volume condensate storage tanks. The hull structure design of condensate storage tanks were provided. Firstly, the column was designed with the double-wall voids (cofferdams ) around the condensate tanks and connected with flats with manholes for maintenance access. Secondly, the pressure head of condensate tank was increased by 10 m compared to the ballast tank after optimizing the reduction of 20 m. Thirdly, the bulkheads of condensate tank are non-watertight and continuous, but with the upper and lower 2 m height gaps for the flowing of condensate and gas. Fourth, the additional corrosion margin of 2 mm was added to the boundary of frequently ballast and de-ballast tanks in the nodes and columns. It is a successful practice for other similar design in the future.

    Objective To explore the incidence rate, severity and risk factors of breast cancer-related lymphedema (BCRL)
    among breast cancer survivors. Methods Data collected from 128 patients underwent breast cancer operation during September
    1 to December 31, 2019. All the patients were female, aging (49.0±6.8) years (29-81 years). The Norman questionnaire and arm
    circumference measurement were used to evaluate the BCRL status and treatment status of breast cancer patients from subjective
    and objective perspectives at 6, 12, 24 and 36 months after surgery separately. The influence of the correlation factors on BRCL
    occurrence was analyzed by Logistic analysis. Results BRCL incidences were 9.4%, 25%, 23.4% and 23.4% at 6, 12, 24, 36
    months according to arm circumference measurement, while 12.5%, 26.6%, 25.0% and 25.0% by Norman questionnaire. Among the 128 patients, 30 were diagnosed with BCRL after breast cancer surgery, among which the incidence of stage Ⅰ
    lymphedema, stage Ⅱ lymphedema and stage Ⅲ lymphedema were 15.6%, 6.3% and 1.6%, respectively. Age, body mass index (BMI)  radiotherapy, the number of positive lymph nodes and surgery method showed differences between the the BCRL group and control group. Both CDT or LVA showed improvement for the BCRL patients. Conclusion BCRL is a common complication for
    breast cancer patients after surgery. It can be fairly diagnosed in the first year after surgery without the increasing incidence of
    BCRL over time throughout 3 years. BMI, radiotherapy were found to be independent risk factors in the development of BCRL in
    this study.

    A decoupling and synchronization control strategy based on a model-compensated extended state observer is proposed in control of a dual-drive gantry positioning stage with asymmetric flexure-linked structures. A gantry dynamic model considering the features of the flexure-linked structures and the impact of load acceleration is built using Lagrangian equations. Translational and rotational control loops are designed according to this model, while extended state observers with compensation of known coupling items in the model are deployed in each control loop to estimate and attenuate the total disturbances composed of unmodeled dynamics, unknown couplings, etc. The experimental results show that the proposed control strategy, which is simple to realize and requires few parameters to be measured, can effectively decouple the dynamics of dual axes and attenuates disturbances, greatly improving the dynamic response and synchronization accuracy of the gantry stage.

    Objective To explore the molecular characteristics of giant congenital melanocytic nevi (GCMN) and their
    clinical significance. Methods The samples of patients diagnosed with GCMN were subjected whole-exome sequencing and
    bio-information analysis to clarify the pathogenic genes and distribution patterns. Results In 50 samples (25 pairs of tissues
    and blood) from 25 patients, 21 patients had somatic mutations, including NRAS gene (19 cases) and BRAF gene (1 case). New
    mutations were detected in 2 patients’
    lesions and blood, and the mutant genes were MET gene and IDH1 gene. In addition,
    no causative mutation was detected in 4 patients. There was no statistic difference between GCMN genotype and phenotype.
    Conclusion NRAS somatic mutation is the most common mutation in GCMN, and BRAF gene has also been detected. Potential
    new mutated IDH1 and MET genes were also found.

    Automated chemical synthesis is one of the long-term goals pursued in the field of chemistry. In recent years, the advent of machine learning (ML) has made it possible to achieve this goal. Data-driven ML uses computers to learn relative information in massive chemical data, find objective connections between information, train models by using objective connections, and analyze the actual problems which can be solved according to these models. With its excellent computational prediction capabilities, ML helps chemists solve chemical synthesis problems quickly and efficiently and accelerate the research process. The emergence and development of ML has shown a strong research assistance in the field of chemical synthesis and characterization. However, there is no highly versatile ML model at present, and chemists still need to choose different models for training and learning according to actual situations. This paper aims to show chemists the best cases of common learning methods in chemical synthesis and characterization from the perspective of ML, such as supervised learning, unsupervised learning, semi-supervised learning, reinforcement learning, etc., and help them use ML knowledge to further broaden their research ideas.

    This paper proposes an interval prediction technology of photovoltaic (PV) power based on parameter optimization of extreme learning machine (ELM) model. First, the weighted Euclidean distance is proposed as the evaluation index of PV power prediction interval. The historical sample units are screened and the ELM training set is optimized. Then, a hybrid optimization algorithm for ELM parameters is proposed. The hidden layer input and output weights and biases parameters of the ELM model are optimized by using the elitist strategy genetic algorithm and quantile regression, and the trained model is used to predict the PV power range. Finally, an actual calculation example is constructed based on the historical data of PV power plants and weather stations. The PV power interval is predicted, and the results are compared with those obtained by other methods. The results of the calculation example show that the method proposed can greatly improve the accuracy of interval prediction while increasing the reliability of interval prediction.

    Extreme meteorological disasters such as typhoons pose a serious threat to the safety of engineering structures. Therefore, the refined simulation on the near-surface atmospheric boundary layer (ABL) is valuable for civil engineering. Large-eddy simulation (LES) implemented in the weather research and forecating (WRF) model has the advantages of multiple options of numerical schemes and high accuracy. It is generally suitable for the refined simulation of the near-surface wind field, although the performance of simulation results is closely related to the numerical methods. This paper assesses the impacts of vital parameters regarding subfilter-scale (SFS) stress models, mesh size, and spatial difference schemes within WRF-LES to simulate the ideal ABL in order to figure out appropriate numerical schemes for the refined simulation of the near-surface wind field. The wind field characteristics are addressed and analyzed such as mean wind speed profile, turbulence intensity profile, and power of spectrum. Comparisons of simulation results among different SFS stress models indicate that the nonlinear backscatter and anisotropy one (NBA1) SFS stress model can effectively improve the accuracy of simulation in the near-surface wind profiles. Investigations of mesh resolution effects indicate that the nonuniformly refined vertical grid near the surface agrees much better with the expected profiles and reduces the expenditure of computational resources. Furthermore, the results show that the even-order spatial difference schemes produce more small-scale turbulent structures than the odd-order difference schemes. The numerical methods of WRF-LES proposed can provide a technical reference for refined simulation of the near-surface wind field and typhoon boundary layer.

At present, the Expendable Conductivity Temperature and Depth is widely used in the field of ocean profile observation because of its characteristics of simple measurement, high measurement efficiency and low hardware cost. In order to understand the development gap at home and abroad and clarify the future development direction of this field, this paper expounds the development status of the expendable marine exploration instruments at home and abroad through comparative analysis and summary starting from its classification, at the same time, compares the main gap between domestic and foreign products, and focuses on the working principle and key technology of the Submarine-launched Expendable Conductivity Temperature and Depth (SSXCTD), meanwhile, the research summarized the future research directions in this field.

    A Structured Pruning Method Integrating Characteristics of MobileNetV3

    Due to its huge amount of calculation and memory occupation, the traditional deep neural network is difficult to be deployed to embedded platform. Therefore, lightweight models have been developing rapidly. Among them, the lightweight architecture MobileNet proposed by Google has been widely used. To improve the performance, the model of MobileNet has developed from MobileNetV1 to MobileNetV3. However, the model has become more complex and its scale continues to expand, which is difficult to give full play to the advantages of lightweight model. To reduce the difficulty of deploying MobileNetV3 on embedded platform while maintaining its performance, a structured pruning method integrating the characteristics of MobileNetV3 is proposed to prune the lightweight model MobileNetV3-Large to obtain a more compact lightweight model. First, the model is trained by sparse regularization to obtain a sparse network model. Then, the product of the sparse value of convolution layer and scale factor of batch normalization layer is used to identify the redundant filter, which is structurally pruned, and experiment is conducted on CIFAR-10 and CIFAR-100 datasets. The results show that the proposed compression method can effectively compress the model parameters, and the compressed model can still ensure a good performance. While the accuracy remains unchanged, the number of parameters on CIFAR-10 in the model is reduced by 44.5% and calculation amount is reduced by 40%.

    Aimed at the short-term extreme value prediction of ship motion, a sliding window method based on motion spectrum information is proposed to extract feature data, based on which, a series prediction model of convolutional neural networks (CNN) and long short-term memory (LSTM) is built. The CNN module aims at the local correlation characteristics of the input data, and the LSTM module aims at the time dimension characteristics of the data. The simulation test results of S175 ship show that the model has a good prediction effect on the motion extremum information in the next 1 and 2 cycles, and the evaluation indexes are significantly better than those of LSTM and gate recurrent unit (GRU) models, which has an important application value.

    Aimed at the joint vibration problem caused by the load change of humanoid flexible joint, a torque compensation control method based on state observer is proposed. By controlling the motor to output a torque increment equivalent to the disturbance torque, the joint torque can quickly balance the load torque and shorten the oscillation process of the elastic element passively adapting to the load change. A state observer for estimating load disturbance torque and motor speed is designed, whose convergence is proved by the Lyapunov function. The control structure of the drive system based on the proportional integral-intergral proportional (PI-IP) speed regulator is established, and the observer output feedforward link is added to the speed regulator to improve the anti-interference ability of the system. The simulation results show that compared with proportional integral differential (PID) control and joint force feedback proportional differential (PD) control, the proposed method can restore the motor speed to stability within 0.6 s after load change and realize joint vibration suppression within 1 s. Besides, the joint speed adjustment time is shortened by about 1.8 s and 0.9 s respectively, which effectively improves the dynamic adjustment ability of the system. Finally, the effectiveness of the proposed method was verified by experiments on an integrated flexible joint testing platform.

    Based on the nonlinear geometric relationship of the bulging angle and the bending angle, and the principle of virtual work and nonlinear constitutive relationship of Neo-Hookean incompressible hyperelastic material, a quasi-static mechanical model for pneumatic toothed soft actuator was established, considering the strain energy of the bottom, side walls, and front and rear walls. Considering the geometric nonlinearity and material nonlinearity, the proposed model could solve the configuration of the soft actuator at different driving pressures and terminal loads precisely and efficiently. The finite element simulation of the cantilevered-free soft actuator was conducted by Abaqus, and the corresponding experimental device was established. The simulation analysis and experimental investigation were performed at different driving pressures. A comparison of the results show that there is a positive linear correlation between the driving pressure and the bending angle of the soft actuator, and the prediction of the theoretical model agrees well with the simulation and experimental results. In addition, the distribution of the strain energy was analyzed. Based on the equal-curvature model, the configuration results of the soft actuator at terminal loads are basically consistent with those obtained by Abaqus. The proposed quasi-static mechanical modeling method provides a theoretical basis for the structural optimization design, performance improvement, and motion control of similar soft actuators.

    Single image deraining aims to recover the rain-free image from rainy image. Most existing deraining methods based on deep learning do not utilize the global information of rainy image effectively, which makes them lose much detailed and structural information after processing. Focusing on this issue, this paper proposes a single image deraining algorithm based on Swin Transformer. The network mainly includes a shallow features extraction module and a deep features extraction network. The former exploits the context information aggregation module to adapt to the distribution diversity of rain streaks and extracts the shallow features of rainy image. The latter uses Swin Transformer to capture the global information and long-distance dependencies between different pixels, in combination with residual convolution and dense connection to strengthen features learning. Finally, the derained image is obtained through a global residual convolution. In addition, this paper proposes a novel comprehensive loss function that constrains the similarity of image edges and regions synchronously to further improve the quality of derained image. Extensive experimental results show that, compared with the two state-of-the-art methods, MSPFN and MPRNet, the average peak signal-to-noise ratio of derained images of our method increases by 0.19 dB and 2.17 dB, and the average structural similarity increases by 3.433% and 1.412%. At the same time, the model parameters of the proposed network decreases by 84.59% and 34.53%, and the forward propagation time reduces by 21.25% and 26.67%.

    The low simulation efficiency of three-dimensional transient calculation makes it difficult to be applied in predicting fuel temperature during the entire fuel consumption process. In order to satisfy the requirements of fuel temperature field calculation in the rapid iterative fuel system design process, a novel efficient and accurate improved thermal network method was proposed. The mixing coefficient was introduced to the heat conduction equation to represent the heat convection inside the fuel, and the heat transfer caused by the mass transfer term was added to the heat balance equation to represent the problem of overall fuel flow. Based on the improved thermal network method, the transient thermal analysis model of the high-speed aircraft fuel tank was constructed. The results show that under different thermal boundary conditions and tank geometries, the temperature deviation between the thermal network method and the computational fluid dynamics(CFD) method is less than 5%, and the calculation efficiency of the thermal network method is much higher, which can greatly improve the computational efficiency. By combining CFD calculation with the improved thermal network method, a novel fuel transient temperature prediction method is provided for the design of the high-speed aircraft fuel storage system.

    “Guidelines for diagnosis and management of hyperthyroidism and other causes of thyrotoxicosis in China” (2022) (referred to as the “guidelines”) was officially published in August 2022 in the Chinese Journal of Endocrine Metabolism. These guidelines have been extensively updated from the 2007 version, with 15 sections covering the etiology, clinical manifestations, diagnosis, evaluation, and treatment of hyperthyroidism, as well as the management of specific types of thyrotoxicosis. The new version guidelines proposed 107 recommended opinions supported by stronger evidence, offering comprehensive and reliable guidance for clinical doctors and medical professionals dealing with thyrotoxicosis. This article aimed to extract the key points and recommended strategies from the guidelines to help clinical doctors better understand and apply the latest guiding principles.

    Objective To explore the relationship between serum N-terminal pro-brain natriuretic peptide (NT-proBNP) level and hydration status and cardiac function indexes in the patients receiving peritoneal dialysis(PD) treatment,and to further analyze the factors affecting the level of NT-proBNP. Methods A total of 111 patients with PD who were followed up in the PD Center of Ruijin Hospital, Shanghai Jiao Tong University School of Medicine from May 2019 to January 2022 were enrolled and the data was analyzed retrospectively. According to the results of multi-frequency bioelectrical impedance body composition monitor (BCM), relative hydration status (DHS), the ratio obtained through dividing extracellular water (ECW) by overhydration (OH), the patients were divided into normal volume group (OH/ECW≤15%) and overhydration group (OH/ECW>15%). The relationship between serum NT-proBNP level and hydration status in two groups was compared, and the related factors affecting the level of NT-proBNP were further analyzed and discussed. Using BCM result as the“gold standard”, the receiver operator characteristic curve(ROC curve) determined the optimal threshold of NT-proBNP as the capacity was overloaded. The patients were divided into two groups according to NT-proBNP and their clinical data were compared. Results The indexes including systolic blood pressure, urine volume, urea clearance index (Kt/V), creatinine clearance rate(CCr), dialysate to plasma ratio for urea at 4 hours(4hD/PCr), types of antihypertensive drugs, type of diuretic, hemoglobin, serum albumin, log2(NT-proBNP), interventricular septal thickness (IVST), left ventricular posterior wall thickness(LVPWT), left atrial diameter(LAD), OH, TBW, ECW, OH/TBW, ECW/TBW and ECW/ICW were detected in volume overload group and normal volume group, and showed statistically significant (all P<0.05). log2(NT-proBNP) level was correlated with systolic blood pressure, dialysis duration, ultrafiltration volume, dialysis dose, antihypertensive drug group, IVST, LVPWT, LAD, left ventricular end-systolic dimension (LVESD), left ventricular end-diastolic dimension (LVEDD), OH, OH/ECW were positively correlated (all P<0.05). It was negatively correlated with urine volume, Kt/V, CCr, normalized protein catabolic rate (nPCR), hemoglobin, serum albumin and LVEF (all P<0.05). NT-proBNP 4 896 ng/L could be used as the critical value of initial screening capacity overload and was applied to divide the patients into two groups, and the systolic blood pressure, urine volume, ultrafiltration volume, Kt/V, CCr, IVST, LVPWT, OH/ECW indicators showed significant differences (all P<0.05) in two groups. Conclusions The level of NT-proBNP in PD patients is generally increased, and it indicates volume overload as its value is greater than 4 896 ng/L, which can be used as a simple monitoring index for the preliminary judgment of volume overload in PD patients.

    Subsea wellheads subjected to complex internal pressure and external loads is the key equipment for deepwater oil and gas development. Currently, stability of subsea wellhead and corresponding supporting tools are mainly assessed by theoretical analysis and numerical simulation. Due to lack of pressure test tools, the actual structure responses of subsea wellhead and corresponding supporting tools could not be reflected. In this work, specialized tools aiming attesting the internal pressure of subsea wellhead and corresponding supporting tools are designed with consideration of different operation conditions and loads. Additionally, for guaranteeing the security of test tools during test experiments, mechanical strength analysis is carried out by establishing related finite element model. Simulation results show that bearing capacity test could be implemented in safety using the designed test tools, and the stress are lower than the yielding strength of test tools. The research could provide technical support for the structural design and optimization analysis of the subsea wellhead test tool.

    Optimization Design of New Bionic Propeller

    A novel method for optimal design of hydrodynamic performance of bionic propeller with a deformable leading edge is proposed. Based on the bionics principle and method of parameterized modeling, the fore-fin concave-convex structure of humpback whales is applied to the propeller leading edge, the leading edge in the propeller to meet flow region according to the exponential decay curve and the standard sine curve smooth leading edge for similar humpback fins protuberant structure of concave and convex deformation, and the leading edge of concave and convex bionic propeller. The hydrodynamic performance, the cavitation performance, and the noise performance of the exponential decay bionic propeller and the sinusoidal function bionic propeller were simulated respectively. The propeller with a better performance is selected, and the simulation based design (SBD) technology is introduced into the optimization design of the new bionic propeller. The parameters controlling the shape of the exponential attenuation curve of the guide edge deformation are taken as optimization design variables, the torque of the parent propeller is taken as the constraint condition, the open water efficiency is selected as the objective function, and the optimization algorithm of Sobol and T-Search is adopted. A bionic propeller optimization system based on the exponential decay curve is constructed. The results show that the application of the concave and convex structure of the humpback whale fore-fin to the guide edge of the propeller improves the cavitation performance and noise performance of the propeller, but the improvement of the open water performance of the propeller is not particularly significant. It is verified that the hydrodynamic performance optimization design method of the bionic propeller established in this paper is effective and reliable, which provides a certain theoretical basis and technical guidance for the performance numerical calculation and configuration optimization design of the bionic propeller.

    DNA methylation detection assists early screening and diagnosis of tumors

    DNA methylation is one of the most widely used indicator for early cancer screening. Under the influence of carcinogens, hypermethylation of the promoter region of tumor suppressor genes may lead to downregulation or silencing of gene expression, thereby activating the expression of proto-oncogenes and promoting tumorigenesis. The samples used for DNA methylation detection are mainly exfoliated cells, blood samples and paraffin-embedded tissues. Commonly used detection methods include: methylation-specific polymerase chain reaction (MSP), nucleic acid mass spectrometry, methylation chip, bisulfite sequencing, next-generation sequencing, etc. Compared with mutation detection, the advantages of DNA methylation detection are that it has higher tumor specificity, more detectable sites, higher signal quality, and can achieve tissue traceability. Currently, it is mainly used clinically for medication guidance for brain glioma, aid in diagnosis and high-risk triage for lung cancer, high-risk triage and recurrence monitoring for bladder cancer, high-risk triage for cervical cancer, and early screening and recurrence monitoring for colorectal cancer and gastric cancer. Before large-scale clinical routine application, accurately positioning of the application scenarios for DNA methylation detection must be considered. For well-tolerated endoscopy, the rigid need of DNA methylation detection for high-risk triage may be reduced. DNA methylation detection plays an important role in assisting in the diagnosis of tumors with low pathological diagnosis sensitivity, tumor monitoring and prognosis assessment. With the standardization of testing procedures and quality management, DNA methylation detection will be more widely used to improve the early screening and diagnostic sensitivity of tumors.

    Current status and considerations on the diagnosis and treatment of pemphigus in China

    Pemphigus is a rare autoimmune bullous disease with chronic and high rates of relapse. The diagnosis of pemphigus in China has following problems: first, the difficulty of early diagnosis commonly causes missed diagnosis or misdiagnosis with eczema, seborrheic dermatitis, behcet disease, ect. Patients are diagnosed clearly only after several months or years. Second, patients’ cognitive dificiency of pemphigus. Shortage of education on pemphigus patients, low acceptance and obedience and self-reduction or-withdrawal of drug causing common relapse and low sensitivity and increasing adverse reactions in retreatment. Thrid, lack of management of doctors in adverse reaction treatment. Some primary hospitals still have the overdosage of hormone to disease control and nonstandard drug combination as the primary treatment of pemphigus. In January 2020, the Dermatology Branch of China Association for the Promotion of International Exchanges in Healthcare has released Expert Recommendations on the Diagnosis and Treatment of Pemphigus Vlgaris (2020) and update the diagnosis standard of pemphigus in China. In April 2021, the China Autoimmune Bullous Disease Collaboration Network was officially established to promote the further update of pemphigus diagnosis and treatment with multi-center and multidisciplinary collaborative research. “Early diagnosis, timely treatment, careful nursing, long-term follow-up” is the principle of pemphigus diagnosis and treatment. This article concludes the situation of pemphigus in China, compares differences between home and abroad in diagnosis and treatment of pemphigus and triggers thoughts of future development.
