    3D printing is used in the manufactory of medical device.This technology could be used in the fabrication of dental medical device,orthopedic and cardiovascular medical device.Because of the uniqueness of this technology,the research of quality control should cover raw material,the process control,facility,data transmission and risk control.Besides the quality control technology,it is important to set up standards.The process control and standardization could improve the risk control and quality of additive manufacturing in medical area.In this paper,the progress of standardization and quality control of additive manufacturing medical device was reviewed.

    A Study of Constructing Tubular Tissue Engineered Cartilage by Using Chondrocytes Cell-Sheet Approach

    Objective To explore the feasibility of constructing tubular cartilage with a diameter similar to goat tracheal in vitro by large mammalian auricular cartilage,based on improved cell-sheet technology.Methods The chondrocyte cell-sheet was cultured for 1-2 weeks in vitro,and was shaped as tubular cartilage tissue.Samples were collected at 6 weeks.The general appearance and elasticity were observed,and the formation of cartilage was examined.Results After cultured for 6 weeks in vitro,the cell-sheet had intergraded to a white translucent tubular tissue.The tubular tissue had certain biomechanical strength similar to mature cartilage tissues,and the histology analysis displayed continuous cartilage phenotype.Conclusion The improved cartilage cell-sheet technology has a series of advantage including limited defect in donor site,short period in vitro culture,less inflammatory response in vivo and so on.This technology would provide a promising approach for the regeneration of cartilage in vivo.

    Mandibular Reconstruction Based on Digital Medicine:Modularized Protocol and Standardization Evaluation

    Objective To introduce a modified protocol of mandibular reconstruction with bone grafts,including modularized protocol and standardization evaluation.Methods From 2013 to 2017,a total of 9 reconstructed mandibles was studied.This program was performed with the following three modifications:computer-assisted surgical simulation(CASS),surgical template(ST)and operation.Standardization evaluation included part comparison analysis(PCA)and mechanical quantitative sensory testing.Results The affected mandible was successfully removed and the inferior alveolar neurovascular bundle(IANB)was conserved.The mean accuracy of PCA was 0.92 mm and the standard deviation was 0.96 mm.Mechanical quantitative sensory testing showed no significant difference of the mental and lower lip sensory function between the affected side and normal side(P=0.12).Conclusion The modified mandible reconstruction program can well repair different types of mandibular defects and retain the mental and lower lip sensory function.

    Digital Technology Assisted in Autologous Costal Cartilage Harvesting and Auricular Scaffold Fabrication

    Objective To explore the feasibility of using digital technology to assist in selective harvesting of costal cartilage and the preparation of ear scaffolds.Methods From October 2016 to August 2017,20 microtia patients accepted ear reconstruction.According to the different physiological characteristics of the costal cartilage and auricle,different number of the costal cartilages were harvested by digital technology navigation for ear reconstruction.Results All the 20 patients completed the operation successfully.The postoperative follow-up was 1.3 years on average.The shape of the reconstructed ear was satisfactory,the incision was uniform,no skin soft tissue necrosis and no cartilage displacement were observed.Conclusion It is a safe and reliable method to apply digital technology to the cutting of costal cartilage,to the design and engraving of the ear.It can improve the precision of autogenous rib cartilage cutting,assist the design and cutting of costal cartilage,and realize the personalized engraving of the ear support.

    Surgical Treatment of Cranio-Orbital Neurofibromatosis with Sphenoid Wing Dysplasia

    Objective To explore the individual surgical treatment strategy for cranio-orbital neurofibromatosis(NF)with sphenoid wing dysplasia.Methods A retrospective analysis was made on the 8 cases with cranio-orbital neurofibromatosis from April 2016 to September 2017.The imaging findings and analysis of the surgical methods and results were summarized.The surgical treatment of this group includes:1 transcranial frontal temporal approach;2 from anterior cranial base to middle skull base and "tunnel" separate technique;3 the technique of temporal lobe repositioned and judgment of the position of supraorbital fissure nerve complex during operation.Results Postoperative pulsatile proptosis and ocular movement function were improved in 8 patients and there was no significant change in visual acuity.All patients were followed up for 7 to 12 months,no significant recurrence was observed.Conclusion Surgical treatment of cranio-orbital neurofibromatosis with sphenoid wing dysplasia is a challenge according to widening of the superior orbital fissure and the displacement of supraorbital fissure nerve complex.However,good results can be achieved through comprehensive preoperative evaluation,reasonable surgical planning,meticulous intraoperative operation and individualized perioperative management.

    The Application of the Indocyanine Green Angiography in the Micro-Circulation Evaluation of Perforator Flaps

    Objective To evaluate the accuracy and safety of indocyanine green angiography in the micro-circulation evaluation of perforator flaps.Methods Indocyanine green(ICG)angiography was carried out in 15 perforator-flap surgeries from July 2014 to May 2018.The evaluation results by traditional methods were compared with the angiography.The final survival of the flaps was compared with the intra-operative angiography results 2 weeks after surgery.Complications were noted.Results ICG angiography detected that 7 flaps had microvascular problems while only 2 cases were discovered by traditional methods.In 3 cases the microcirculation problem could not be corrected during the surgery.Partial necrosis took place in all 3 cases and the necrosis area matched the non-perfusion area in the intra-operative ICG angiography.Conclusion ICG angiography is accurate in perfusion evaluation of the perforator flaps.It is more sensitive to the microcirculation problems than traditional evaluation methods.It can effectively reduce the incidence of partial necrosis.ICG angiography has practical value in perforator flaps surgeries.

    Objective To explore the relationship between blood perfusion and pulsed dye laser(PDL)effectiveness of port-wine stains(PWS)on extremities.Methods Patients with untreated PWS on extremities were selected.Blood perfusion of the selected sites on both proximal and distal limbs as well as control sites was detected by laser speckle image(LSI)before treatment.Then the patients were treated with PDL for 3 sessions in total.After 3 months of treatment,the therapeutic effect was evaluated,and the relationship between blood perfusion and therapeutic effect was analyzed.Results Nineteen patients were included,17 on upper limb and 2 on lower limb.The mean Speckle Flow Image(SFI)value of the PWS on upper arm/thigh was significantly lower(80.51 ±16.96)PU than the SFI on hand/feet(155.68 ±71.86)PU.According to photometric evaluation and VAS score,the PDL efficacy on proximal limbs was significantly better than distal limbs(P<0.05).Significant correlations existed between blood perfusion and PDL efficacy(r=-0.351,P=0.031).Conclusion PWS on the proximal limb responds better to PDL,compared with PWS on the distal limb.This variation in efficacy may attribute to differences in blood perfusion.

    A Comparative Study of the Efficacy of Volar Locking Plate and External Fixator in Treating Distal Radius Type C Fracture

    Objective To compare the efficacy of volar locking plate and external fixator in treating distal radius type C fracture.Methods A randomized controlled study was used.Patients with distal radius type C fractures from October 2014 to October 2017 were included and randomly divided into 2 groups:external fixator group and volar locking plate group,treated with external fixator and volar locking plate respectively.The duration of operation,intra-operative blood loss,fracture healing time and incidence of postoperative complications were compared between the two groups.The wrist joint activity,radial inclination,palmar tilt,the ratio of the grip strength with two hands and the Gartland-Werley score of the wrist joint at the last follow-up were measured and compared.Results A total of 30 patients were included and followed up for 1 year.There were 14 cases in external fixation group and 16 cases in volar locking plate group.The duration of operation in the volar locking plate group was longer than external fixator group and the blood loss was more.At the last follow-up,the palmar tilt and radial inclination of the volar locking plate group were better than external fixator group.The fracture healing time,wrist flexion and extension,wrist rotation,the grip strength ratio with two hands and the Gartland-Werley score of the wrist joint had no significant difference between the two groups.No postoperative complications occurred in the two groups.Conclusion The operative risk of external fixator is less than volar locking plate in treating distal radius type C fracture.External fixator can cause reduction loss after operation at a certain degree.However,the loss of the reduction does not affect the recovery of wrist joint function.

    A Prospective Study of the Early Application with a New Model of Adjustable Support Protector in Penis Reconstruction

    Objective To explore the early application of adjustable support protector in penis reconstruction.Methods From January 2015 to July 2017,30 cases received penis reconstruction were selected and divided into the experimental group(n=15)and the control group(n=15).The control group was given the thick cotton pads after operation,and the experimental group was given the adjustable support protector to replace the heavy cotton pad.The survival rate of the penis flap,the accuracy of the fixed angle of the penis and the sensory function improvement were compared between the 2 groups after operation.Results The survival rate of the penis flap,the accuracy of the fixed angle of the penis,the infection rate and the sensory function improvement in the experimental group were significantly better than those in the control group after operation(P<0.05).Conclusion The adjustable brace can effectively protect the skin flap in early stage after penis reconstruction.The effect is better than heavy cotton pad,which is more conducive to early rehabilitation after operation.

    Objective To explore the clinical efficacy of the continuous nursing care based on internet after hypospadias repair.Methods From January 2016 to December 2017,104 cases with hypospadias were retrospected,according to the different postoperative nursing method,the patients were divided into two groups:control group(n=52)and observation group(n=52).The patients in control group were treated with routine nursing care,while the patients in observation group were treated with continuity nursing based on internet.The clinical nursing effects of the two groups were compared,including the pain degree,the postoperative complications,wound healing and duration of postoperative indwelling catheter.Results Compared with control group,the pain degree,complication rate and duration of postoperative indwelling catheter in observation group were all lower(P<0.05),while the wound healing rate was higher(P<0.05).Conclusion The continuous nursing based on internet using after hypospadias repair can achieve significant nursing effect,and it is worthy of clinical promotion.

    The Application of Compound Lidocaine Cream Combined with Physical Cold Compress in Facial Hyaluronic Acid Injection

    Objective To explore the effect of topical anesthetic compound lidocaine cream combined with ice compress on facial hyaluronic acid injection.Methods A total of 60 patients who were scheduled to undergo facial hyaluronic acid injections between April 2017 and October 2017 were selected and divided into two groups:experimental group and control group(n=30).Patients in experimental group were treated with topical compound lidocaine cream before injection and ice compress after injection,while the patients in control group were treated with ice compress after injection.The pain score and pain relief of the two groups were compared.Results The pain score of the experimental group was significantly lower than that of the control group,and the pain relief level was significantly higher than that of the control group(P<0.05).Conclusion The topical anesthetic compound lidocaine cream combined with physical cold compress in facial hyaluronic acid injection has a significant analgesic effect and is simple and easy to carry out,which is worth promoting.

    Natural polymer gelatin(GT)and synthetic polymeric materials polycaprolactone(PCL)have good biocompatibility and biodegradability,which could be used in tissue engineering.However,the disadvantages of simple component GT or PCL limits their application in tissue engineering research.GT/PCL composite scaffold overcomes the shortcomings of the simple component,and has better physical and chemical characteristics with the complementary advantages of both components,therefore has been widely used in the research of producing tissue-engineered organs and repairing tissue damages.In this paper,the characteristics of GT/PCL composite electrospun nanofibrous scaffold and its applications in many fields of tissue engineering were systematically reviewed.
