    The Molecular Mechanism and Application of miR-26a in Bone Regeneration

    The ideal means to repair large-sized bone defects is to induce bone regeneration. MicroRNAs (miRNAs), as a modulator at a post-transcriptional level, can regulate the expression of a series of target genes. A number of studies have shown that miR-26a could promote the expression of multiple key growth factors related to osteogenesis and angiogenesis, and further improve the osteogenic differentiation of mesenchymal stem cells and bone formation. Clarifying the molecular mechanism of miR-26a in promoting osteogenic differentiation of stem cells could provide potential targets for bone regeneration. The mechanism and current progresses of miR-26a in promoting osteogenic differentiation and bone regeneration were reviewed.

    Stromal vascular fraction (SVF) are the remaining cells of the ingredients after removing mature fat cells in the adipose tissue of transplantation. In addition to containing a certain amount of adipose derived stem cells (ADSC), SVF also includes many other cells, which may all have the potential of promoting angiogenesis. In this paper, the role of SVF in angiogenesis after fat transplantation was summarized by reviewing relative literature in recent years. According to the literature, angiogenesis and fat graft revascularization are regulated by various factors: SVF promotes secretion of a diverse array of cytokines and growth;SVF differentiated to pericytes has the function of stabilizing endothelium vascular network;As components of SVF, adipose derived stem cells (ADSC) have the potential of multi-directional differentiation, and macrophages can promote local angiogenesis in hypoxia environment. These results may provide directions of improving the survival rate of fat cells in clinical autologous fat transplantation.

    The research showed that the dendritic epidermal γδ T cells (DETCs) found in murine skin expressed aγδTCR composed of Vγ3Jγ1-Cγ1and Vδ1Dδ2Jδ2-Cδ chains which specifically expressed in murine skin. Activated DETCs produce several cytokines, growth factors and chemokines. All these factors were related to tissue repair, cell survival, proliferation, migration, recruitment, and play a key role in maintaining the epithelium integrity and repairing the epithelial tissue. In this paper, the functions of DETCs in maintaining the epithelium integrity and would repair were reviewed.

    Picosecond laser is known for its extremely short pulse width, through photomechanical and photothermal effects, which could be highly efficient on pigment particles. Currently, picosecond laser is mainly used in tattoo removal, and also effective for pigmented lesions treatment and facial rejuvenation. But the research of these two fields is lack, and the evidence is not strong enough. More studies are needed to prove its efficacy.

    Objective To explore the effect of 3D printing technique in clinical PBL teaching of complex intertrochanteric fractures in order to improve the overall quality of orthopedic teaching. Methods A total of 28 students were included in the orthopedic clinical program. The random number method was used and the students were divided into 2 groups:traditional teaching method group and new teaching method group (3D printing technique, PBL teaching method). The teaching method was explained by the entity, and then the procedure was followed. The effects of the two groups of teaching methods were compared, including the student's academic performance, comprehensive ability self-assessment data. Results Compared with the traditional teaching method group, the scores of the new teaching method group were higher, and the difference was statistically significant (P<0.05). Conclusion The new teaching method of PBL teaching method with 3D printing technique has good teaching effect in clinical teaching of complex femoral intertrochanteric fractures.

    Syndactyly is one of the most common congenital hand differences, characterized with certain degrees of fusion of adjacent digits in soft tissues or bones. It can be associated with polydactyly, camptodactyly, brachydactyly or symphalangism. Diversity of manifestations and incomplete penetration of heritage make the diagnosis and treatment lack of a clear routine. The management of syndactyly was based on the individual experience over the past years with shortage of the knowledge in genetics and surgical strategy. Therefore, to standardize the treatment and update the surgical procedure in syndactyly, to decrease the huge waste of medical resources, the Hand Surgery Branch of Chinese Medical Association, Chinese Medical Doctor Association and Chinese Association of Rehabilitation Medicine convened an expert panel specialized in syndactyly with 10 famous domestic medical colleges and affiliated hospitals, to discuss and formulate about the spectrum of diagnosis and treatment. The consensus statement on diagnosis and clinical management of syndactyly was established for clinical references.

    Long-Term Clinical O bservation of Umbilical Cord Mesenchymal Stem Cells in Treating Spinal Cord Injury by Intrathecal Injection

    Objective To observe the clinical efficacy and safety of umbilical cord mesenchymal stem cells (UC-MSCs) in treating spinal cord injury (SCI) by intrathecal injection. Methods From January 2008 to October 2013, 35 admitted patients with SCI were given UC-MSCs by intrathecal injection for 46 courses, at a dose of 1×106 cells/kg body weight once a week, 4 times as a course. American Spinal Injury Association (ASIA) standard and International Association of Neurorestoratology SCI Functional Rating Scale (IANR-SCIFRS) were used to evaluate SCI patient's neural function and ability of daily living. Results Among 35 cases, 22 cases were effective. Compared with the scale before therapy, effective patients'algaesthesis, tactile sensation, motion, activity of daily living scale were obviously increased one month after therapy (P<0.01). Nine effective patients received 2 to 3 courses of treatment, the effects of 7 cases were further enhanced. The effective rate of 25 cases with incomplete SCI was 88.46%, whereas all of 10 cases with complete SCI had no response. After therapy, there were headache, lumbago or fever in individual patients and disappeared within 1-3 days. All patients were followed up for 45 months to 114 months (average 84 months), no treatment-related adverse events happened. The effect was stable in effective patients. Conclusion UC-MSCs therapy by intrathecal injection is safe, and can ameliorate most of incomplete SCI patient's neurological function, multiple courses of treatment is likely to further improve the efficacy of patients.

    Objective To explore the effect of basic fibroblast growth factor (bFGF) on the proliferation and chondrogenesis of human bone marrow mesenchymal stem cells (hBMSCs), and to determine the cell generation schedule suitable for in vitro cartilage construction of hBMSCs. Methods hBMSCs were obtained and cultured respectively in two culture systems:DMEM and DMEM+bFGF. Two groups of cells were repeatedly passaged to P4 generation, the morphological changes and cell yield rates of hBMSCs were compared between the two groups. At the same time the two groups of P3 cells were induced for chondrogenic differentiation using pellet culture for 3 weeks in vitro. Gross observation, immunohistochemical staining of collagen Ⅱ were used to evaluate the results of each group. Based on the study mentioned above, the DMEM+bFGF culture group was subcultured to P8 to observe the morphological changes of each subculture. After chondrogenic differentiation using pellet culture for 3 weeks in vitro, gross observation and immunohistochemical staining of collagenⅡwere evaluated as before. Results The cell morphology, cell yield and cartilage differentiation in DMEM+bFGF group was far greater than that of DMEM group. Under the culture system of DMEM+bFGF, cells passaged at 1:3 could still maintain a good cell morphology in P6 generation. Immunohistochemistry showed that cartilage-specific extracellular matrix typeⅡcollagen was expressed better in P1-4 generation. Conclusion bFGF can significantly promote the proliferation andchondrogenesis of hBMSCs. P1-4 generation cells in DMEM+bFGF culture system is suitable for in vitro cartilage construction.

    Effects of Complex Decongestive Therapy Combined with Manual Lymphatic Drainage in the Treatment of Lymphedema of Limbs

    Objective To observe the clinical efficacy of complex decongestive therapy (CDT) combined with manual lymphatic drainage in the treatment of lymphedema of limbs. Methods From June 2014 to June 2015,102 cases of limb lymphedema with different etiology and location were treated by CDT combined with manual lymphatic drainage for 1 course (4 weeks). Multiple frequency bioelectrical body composition analyzer was used for monitoring the extent of tissue edema and human BMI changes, and the limb circumference changes were measured. Subsequent CDT self -treatment was ordered. Results The limb edema degree, BMI index and limb circumference were all significantly decreased after CDT treatment (P <0.001). The variation of water content, circumferential diameter and BMI before and after the treatment were all significantly correlated (r=0.445, 0.567, 0.572; P<0.001). All patients were followed up for 6 months, 90% patients insisted on CDT self-treatment, and no obvious recurrence was observed. Conclusion CDT treatment can significantly eliminate the lymphedema of extremity edema and reduce BMI, and subsequent CDT self-treatment is also indispensable.

    The Shape Improvement of Double Eyelid after Modified Double Eyelid Blepharoplasty

    Objective To summarize the long-term outcome after modified double eyelid blepharoplasty. Methods On the basis of the traditional upper blepharoplasty, using Park method of blepharoplasty to retain the orbicularis oculi muscle of the inferior incision margin above the tarsus, which can be firmly tied down with the superior margin of the tarsus. Results All 67 patients were treated with this method, and followed up for 1 to 1.5 years. All patients were satisfied with the long-term outcome. No complication was observed. Conclusion The modified double eyelid blepharoplasty can keep the connection of the orbicularis oculi muscle anatomically, which can reduce the upper eyelids edema rapidly and make the radian of the double eyelids natural and longer-lasting. So this technique is worthy of application and promotion in clinic.

    Clinical Efficacy of Surgical Correction for Inverted Nipple

    Objective To observe the clinical efficacy of surgical correction for inverted nipple. Methods The clinical data of 26 cases with nipple rectification were retrospectively analyzed. Fifteen cases were treated with self-made fixator traction method, and others were treated with tissue flap method. The clinical efficacy of the two groups were observed, and the postoperative nipple retraction rate was compared between the two groups. Results No incision infection and nipple areola ischemic necrosis were observed. Good appearance and normal nipple feeling were achieved. All patients were followed up for 6 months, only one case showed a mild retraction of nipple, while the rest was not significantly altered. There was no significant difference of postoperative nipple retraction between the two operation methods (P>0.05). Conclusion The clinical effect of surgical correction for inverted nipple is satisfactory, and there is no significant difference of clinical effect between the two types of surgical methods.

    Effectiveness of Targeted System Nursing on Improving the Postoperative Effect of Minimally Invasive Laser Eye Bag Plastic Surgery

    Objective To explore effective perioperative nursing measures for minimally invasive laser pouch plasty. Methods From March 2014 to April 2016, 64 cases with eye bag were selected and randomly divided into experimental group (n=32) and control group (n=32). All patients were treated with super pulse CO2 laser method. The patients in control group received routine nursing care after operation, while the patients in experimental group received targeted systematic nursing based on the routine nursing. The satisfaction rate of the nursing and the complications of the two groups were compared. Results In the evaluation of nursing satisfaction, the satisfaction rate of the control group was only 81.25%, which was significantly lower than that of the experimental group (96.88%). The incidence rate of postoperative complications in the control group (28.13%) was significantly higher than in the experimental group (6.25%). Conclusion In the perioperative period, targeted systematic nursing is very important to improve the treatment effect and the satisfaction of nursing work, and can reduce the incidence of postoperative complications, which is worthy of clinical application.

    Objective To compare the clinical efficacy of DHS plus hollow screw and PFNA internal fixation in the treatment of intertrochanteric fractures in the elderly. Methods From June 2014 to September 2016, 80 elderly patients with intertrochanteric fractures admitted to our hospital were retrospectively analyzed. Among them, 40 cases were treated with DHS plus hollow screw (DHS group), while the other 40 cases were treated with PFNA internal fixation (PFNA group). The clinical data, operation conditions, hospitalization days, Harris score and postoperative complications in both groups were analyzed and compared. Results ①There was no significant difference between the two groups in terms of gender, age and fracture classification. ② The average incision length, operation time, intraoperative blood loss, post-operative drainage, weight-bearing time, fracture healing time of the cases in the PFNA group were significantly better than those of the cases in the DHS group;③There was no significant difference in the aspects of Harris score and postoperative complications between the two groups. Conclusion PFNA internal fixation is more suitable for elderly patients with intertrochanteric fractures, which is worthy of further promotion.

    Objective To discuss the effects of reconstructing clavicular ligament with loop plate combined arthrodesis on the recovery of shoulder function for acromioclavicular joint dislocation (AJD). Methods From January 2015 to January 2017, 68 patients with AJD were selected. According to the random distribution, all patients were divided into loop plate group (n=34) and hook plate group (n=34). Patients in hook plate group were treated with clavicular hook plate combined with ligament repair, while patients in loop plate group were treated with clavicular ligament reconstruction with loop plate combined with arthrodesis. The operation time, bleeding, hospitalization days and cost, the complications, Constant-Murley shoulder function score (CM score) and shoulder function recovery were compared between the two groups. Results The operation time, intraoperative bleeding, hospitalization days and cost between loop plate group and hook plate group were compared, and the difference was not statistically significant (P>0.05). The complication incidence in loop plate group was significantly lower than that in hook plate group (P0.05). Conclusion Reconstructing clavicular ligament with loop plate combined arthrodesis is more suitable for the biomechanical requirements of acromioclavicular joint, it can effectively reduce the complications of AJD patients and promote shoulder function recovery, so it's worth of further clinical promotion.

    The Research Progress and Application of Human Urine Derived Stem Cells in Oral Tissue Engineering

    Human urine-derived stem cells (hUSC) have been reported, and regarded as a candidate for seed cells in tissue engineering, which are highly expandable, and have self-renewal capacity, multi-differentiation potential and paracrine properties. More importantly, hUSC can be obtained via a non-invasive, simple way, and are low immunogenic without medical ethics restrictions. hUSC as an ideal source of stem cells, have demonstrated a prosperous progress in scientific research and potential application in the field of tissue engineering and regenerative medicine. The research achievement in urinary system, bone tissue engineering also implies potential applications of hUSC in oral maxillofacial tissue regeneration. In this paper, the application of hUSC in oral tissue engineering and regeneration were reviewed by summarizing domestic and overseas research progress on hUSC.

    Clinically, the bone defects may be caused by acutely trauma and bone tumor resection. In this situation, the self-repairing capability could not be enough for bone regeneration. The materials for bone regeneration are used for filling bone defects and induce bone repair. In the medical device registration regulation, these materials are belong to the class Ⅲmedical device. There are several products which had registration license of CFDA. In this paper, the materials used for the production of bone inducing products were summarized, including artificial inorganic materials, artificial polymer materials, natural polymer materials and composite materials.
