    The Effect of PRP on Proliferation of Human Skin Fibroblasts and Expression of Collagen and Hyaluronic Acid in Vitro

    Objective To explore the effect of platelet-rich plasma(PRP) with different concentrations on proliferation of human skin fibroblasts and expression of collagen and hyaluronic acid(HA) in vitro.Methods Primary cultured human skin fibroblasts(HSFs) with different concentrations(1%、5%、10%、20%、30%、50%) of PRP culture medium were treated as experimental groups,and take serum-free medium as negative control group and culture medium containing 10% newborn calf serum as positive control group.MTT,RT-PCR and ELISA were respectively used to measure HSFs proliferation,collagen expression and hyaluronic acid expression at the 3rd,5th and 7th day.Results MTT assay indicates that the experimental groups can all promote the proliferation of HSFs compared with the negative control group(P<0.01),and the effect showed on increasing tendency.RT-PCR detection showed that the target genes Collagen Type 1(COL1) and Collagen Type 3(COL3) were all expressed in the 50% PRP group,negative control group and positive control group,but there were no significant differences among the three groups.ELISA showed the enhancement of HA expression compared with the negative control group(P<0.01).The HA expression was significantly increased with the PRP concentration of 30%-50%(P<0.01).Conclusion PRP could promote the proliferation of HSFs and the function of the synthesis of collagen and HA.

    Objective To investigate the effects of P17-BMP2 peptide on the proliferation and osteogenic differentiation of bone marrow mesenchymal stem cells(BMSCs).Methods BMSCs derived from 4-week-old Wistar rats were isolated and expanded.Four groups were set in this study: Group A,which were cultured in regular medium(DMEM with 10% fetal calf serum);Group B,which were cultured in the regular medium with dexamethasone 107 mol/L,Vitamin C 10 mg/L and beta-glycerophosphate 10 mmol/L(induction medium);Group C,which were cultured in induction medium with P17-BMP2 10 μg/mL;and Group D,which were cultured in regular medium with P17-BMP2 10 μg/mL.Cell proliferation were measured at 1、3、5、7 days after culture by CCK-8 kit.Alkaline phosphatase(AKP) staining were performed 7 days after culture.The expressions of osteocalcin(OCN),osteopontin(OPN),runt-related transcription factor 2(Runx2) were evaluated by Q-PCR.Results Compared with group A,the proliferation of BMSCs were not promoted in group D,but promoted in group B and highly enhanced in group C.The alkaline phosphatase staining were all positive in each group,but the dyeing intensity were different: group C > group B > group D > group A.Compared with group A,the expressions of OCN,OPN and Runx2 were

    Screening of siRNA Targeted PAR-2 in Keratinocyte

    Objective To design and screen small interfering RNA(siRNA) targeted protease-activated receptor-2(PAR-2) mRNA in Keratinocyte that can specifically and effectively suppress PAR-2 expression.Methods Three pairs of PAR-2 specific double stranded siRNAs were designed according to the sequence of PAR-2 mRNA.These siRNA were transfected into human keratinocyte by Lipofectamine 2000.The transcription level of PAR-2 was analyzed by semi-quantitative RT-PCR and PAR-2 protein expression was tested by western blot in order to evaluate the effect of RNA interference.Results Two of the three PAR-2 siRNA effectively inhibited the expression of PAR-2 and the second one is more effective.Conclusion PAR-2 siRNAs can be successfully designed and synthesized,which can specifically and effectively suppresses the expression of PAR-2 gene.The results in this study lay the foundation for further study on PAR-2 gene silencing by RNA interfere(RNAi).

    Effect of Tanshinone ⅡA on Cell Proliferation,Apoptosis and Cell Cycle of Fibroblasts Derived from Keloid

    Objective To explore the effect of Tanshinone ⅡA on cell proliferation,apoptosis and cell cycle of fibroblasts derived from keloid.Methods Fibroblasts derived from keloid were cultured with different concentration of Tanshinone ⅡA(0 μg/mL,50 μg/mL,100 μg/mL and 200 μg/mL) in vitro.CCK-8 was used to detect cell proliferation,and flow cytometry was used to analyze cell apoptosis,cell cycle at the different time.Results Cell proliferation of fibroblasts derived from keloid was decreased,apoptosis at the early phase was increased,and cell cycle in G0/G1 phase was arrested by different concentration of Tanshinone ⅡA with different intervention time(P<0.001),especially in 200 μg/mL group.Variance analysis showed that those differences have statistical significance.Conclusion Tanshinon ⅡA can suppress cell proliferation,induce cell apoptosis and arrest cell cycle of fibroblasts derived from keloid.The effect of Tanshinone ⅡA was affected by different concentration and different intervention time.

    Cartilage-Derived Morphogenetic Protein 1 in the Induction of Chondrogenic Differentiation of Scar Fibroblasts in Vitro

    Objective To investigate the feasibility of the differentiation of scar fibroblasts into chondrogenic phenotype in vitro induced by Cartilage-derived morphogenetic protein 1(CDMP1) for the reconstruction of ear defect.Methods Scar fibroblasts were extracted from hypertrophic scar and cultured in momo-layer in vitro.Scar fibroblasts of the 4th passage were induced by CDMP1(100 ng/mL in medium of F12+2%FBS).Negative and positive control groups were also set up.After 7 days of induction,morphological change of cells was observed by Image Plus software.Expression of col Ⅰ,col Ⅱ,sox 9 and aggrecan were detected by immunofluorescence,RT-PCR and western blot.Micromass cultured scar fibroblasts were induced and cultured.The results were analyzed by HE staining,immunohistochemistry and Safranine-O staining.Results After induction,the morphology of the cells changed from spindle to polygon shape.By Image Plus software analysis,the length/width ratio changed from(7.21±1.54):1 of pre-induction to(1.48±0.21):1 of post-induction(P<0.05).No significant difference was found between post-induction fibroblasts and normal chondrocytes(P>0.05).Col Ⅱ,sox9,aggrecan were expressed in post-inductive cells by immunofluorescence,RT-PCR and western blot.HE staining,immunohistochemistry and Safranine-O staining revealed cartilage specific structure and deposition of cartilage extracellular matrix in micromass cultured scar fibroblasts.Conclusion Scar fibroblasts could be differentiated into chondrogenic phenotype by CDMP1 induction and induced scar fibroblasts can be used as the seed cells for the reconstruction of ear defect.

    The Natural Course of Salmon Patch and the Identification between Salmon Patch and Port Wine Stain

    Objective To explore the natural course of salmon patch and the identification between salmon patch and port wine stain.Methods Between March 2007 to May 2011,thirty-six patients with face and neck stains were analysed.The locations and changes of the stains were observed to conclude the differences between salmon patch and port wine stain.Results Twenty-one patients were diagnosed salmon patch and 15 patients were involved in multi-region(71.4%).Among them,forehead and glabella were involved in 16 cases(38.1%),while nape,eyelid and phitrum in 13,9 and 3 cases respectively(31.0%,21.4% and 7.1%).Eighteen stains were fade away at the age of 2(85.7%).The other 3 patients were followed up and the stains had no change,so the diagnosis was changed as port wine stain.Fifteen port wine stains were diagnosed,including 2 cases in the temproal region(13.3%),8 cases in the cheek region(53.3%),3 cases in the mandible region(20%).Two patients involved in several regions(13.3%).90% of the stains had no obvious change at the age of 2.The stains of the other 2 patients were slightly darken.Conclusion Salmon patch can be confused with port wine stains in shape.But they are different in locations and natural courses.Long standing follow up will aid in correct diagnosis and appropriate management of these birthmarks.

    Pulsed Dye Laser in the Treatment of Ulcerated Hemangioma in Infancy

    Objective To explore the clinical efficacy and safety of pulsed dye laser for the treatment of ulcerated hemangiomas.Methods Children with ulcerated hemangiomas were enrolled in the outpatient department.All patients with clean wound were treated with Cynosure pulsed dye laser(wave length: 585 nm;pulse width: 0.45 ms;spot size: 5 mm;energy density: 6.0-6.5 J/cm2).Lesions were treated in a sequential pattern at 2-3 week intervals until cutaneous healing.The time of reepithelialization,pain relief and side effects such as bleeding and infection were evaluated after laser procedure.Results Fifteen infants with ulcerated hemangiomas were included.All patients didn’t received other treatments before and during the laser procedure.Fourteen of 15 patients(93.3%) required laser treatment for one time with the healing time of ulceration ranged from 1 to 2 weeks.Only one patient(6.7%) required two times of laser treatment.The average time of complete ulceration healing was(1.68±0.23) weeks.Pain was controlled within 1-2 days after the laser treatment in all patients.Bleeding,infection and ulceration worsening were not observed.Conclusion Low-fluence pulsed dye laser is safe and effective for the treatment of ulcerated hemangiomas.It can accelerate the healing of ulceration and release the pain.

    Clinical Application of Flap Pedicled with Medial Plantar Artery Branch

    Objective To explore the design and clinical application of flap pedicled with medial plantar artery branch.Methods Five cases with palmer soft tissue defects and six cases with finger pulp wound were repaired by medial foot flap and medial plantar flap based on the superficial and deep branch of the medial plantar artery respectively.The flap was 3 cm × 3 cm to 10 cm × 7 cm in size.The skin of donor site was repaired by full-thickness skin graft.Results Eleven cases of all flaps were survived and healed completely.Skin necrosis of donor site was observed in 1 case and healed under scab after dressing.Ten cases were followed up for 6 to 18 months.The flap texture was good and no bloated appearance was observed.Two-point discrimination was measured from 7 mm to 9 mm.After functional rehabilitation,the functional results were excellent in 5 cases,good in 3 cases,fair in 2 cases according to Chinese Medical Association Hand Surgery Function Assessment.Conclusion Flap pedicled with medial plantar artery branch is an ideal choice for repairing palmer and finger pulp wound with satisfied blood supply,shape and texture.

    Arthroscopic Micro-fracture for the Treatment of Talus Cartilage Injury in 60 Cases

    Objective To explore the efficacy of arthroscopic micro-fracture in the treatment of talar cartilage injury.Methods Sixty cases with talar osteochondral injury were operated by arthroscopic micro-fracture method.The changes of ankle function score and pain score were observed.Results The ankle function score has an average increase of 20.1 points after the treatment and significant difference was observed(P<0.05).The pain score has an average increase of 5.1 points after the treatment and significant difference was observed(P<0.05).Conclusion Arthroscopic micro-fracture has significant clinical efficacy in the treatment of talar osteochondral injury and is worthy of popularization.

    Effects of Single-Bundle and Double-Bundle Reconstruction in the Repair of Anterior Cruciate Ligament under Arthroscope

    Objective To compare the clinical effects of arthroscopic single-bundle and double-bundle reconstruction in the repair of anterior cruciate ligament(ACL).Methods Thirty-one patients with ACL rupture were operated by double-bundle reconstruction while thirty patients with ACL rupture were operated by single-bundle reconstruction under arthroscope.Joint stability test,Tegner score and postoperative satisfaction were evaluated.Results Lachman test were positive in 1 case of double-bundle group and 7 cases of single-bundle group(P<0.05);Edge to edge differences were less than 3 mm in 27 cases of double-bundle group and 19 cases of single-bundle group(P<0.05);Tegner score of double-bundle group was better than single-bundle group.Conclusion Double-bundle reconstruction could fit the space structure of normal knee ACL and could better satisfy the biomechanical require of knee joint at unbend and reverse position.

    A Comparative Study of Bone Fusion for Lumbar Spondylolisthesis by Three Methods

    Objective To compare the therapeutic effect of posterolateral lumbar fusion(PLF),posterior lumbar interbody fusion(PLIF) and trans-foraminal lumbar interbody fusion(TLIF) for lumbar spondylolisthesis.Methods According to three different fusion methods,120 patients with lumbar spondylolisthesis were divided into 3 groups: group A(PLF,n=36);group B(PLIF,n=42) and group C(TLIF,n=40).After the pedicle screws-rod internal fixation,all the patients were operated by different fusion methods.Clinical outcome and fusion rates of the three groups were reviewed and compared.Results Operation time and blood loss in group C were much lower than group A and B(P<0.05),but bone fusion rate in C group was much higher than group A and B(P<0.05).The good rate of group A,B and C were 77.8%,83.3% and 95.0% respectivly,but there was no statistically significant difference among the three groups(P>0.05).Conclusion Pedicle screws-rod internal fixation is a better way to raise bone fusion rate and reduce complications.TLIF with shorter operation time,lower blood loss,larger fusion area and higher bone fusion rate,is a good method for the treatment of spondylolisthesis.

    Frontal Muscle Flap Suspension in Recreating the Eye Aesthetic Characters in the Patients with Poor Upper Lid Ptosis Correction

    Objective To explore the feasibility of the frontal muscle flap suspension in recreating the eye aesthetic characters and the 3 dimensional structure in the patients with poor upper lid ptosis correction.Methods Seven cases of patients with poor upper lid ptosis correction were operated by frontalis muscle flap suspension to recreate the eye aesthetic characters.After operation,the following contents were evaluated: upper eyelid sulcus,crease increase of upper eyelid,compensatory shift of ipsilateral eyebrow and mild drooping of upper eyelid.Results The follow-up period was 3-6 months.Six patients obtained obvious results and one patient thought the result is acceptable.No surgical complications was observed.Conclusion Frontalis muscle suspension technique is available for patients with poor upper lid ptosis correction to restore their ocular aesthetic structure.

    Autologous lipo-transfer has been widely used in soft tissue implantation,yet its clinical application has the problem of instability of local survival rate.Recent data demonstrated that varies types of stem cells and platelets may promote the formation of new blood vessels of ischemia tissue through a variety of mechanisms.It plays an important role in promoting the survival of fat tissue and reducing the local atrophy of fat transplantation.Application of cell therapy in autologous lipo-transfer could enhance long-term outcomes,avoid absorption of adipose tissue after transplantation and reduce local complications.

    Craniosynostosis is a kind of common congenital craniofacial deformity,which represented the early close of one or more cranial sutures.Several factors can affect the growth of cranial vault during embryonic life and after birth,leading to different types of craniosynostosis.It has been showed that growth factors play crucial roles in cranial suture fusion.Research progress of growth factors in craniosynostosis is reviewed in this paper.

    In 1970,the first bone substitute-PMMA was created and a lot of clinical studies were conducted.Clinical applications demonstrate good results and the mortality rate of osteomyelitis is significantly reduced.According to the degradation of materials,the drug-delivery system can be divided into biodegradable materials and non-degradable materials.As the bone substitute,the two types of materials were required to achieve the minimum inhibitory concentration in nature.The system types,preparation methods,the nature of the drug,and the drug release kinetics were reviewed.
