    In Vitro Study of Differentiation of hADSCs into Epidermal Cell Linage Induced by EIF-GT/PCL Electrospun Nanofibrous Membrane

    Objective To study the biological characteristics of epidermal inducing factors(EIF)-gelatin(GT)/polycaprolactone(PCL)electrospun nanofibrous membrane and its ability of inducing hADSCs into epidermal cell linage.Methods EIF-GT/PCL electrospun nanofibrous membrane(experimental group)was produced by blending electrospun technology,and the GT/PCL electrospun nanofibrous membrane was produced as control group.Their morphology and mechanical characteristics were observed.The accumulated EIF releaserate-time curve was studied using ELISA.The hADSCs were seeded onto the membrane of the two groups and were cultured in vitro.Scanning electron microscope(SEM)was used to observe the cell-membrane adhesion.The hADSCs proliferation on was measured by CCK-8 and their differentiation into epidermal cell linage was confirmed by immunofluorescence of CK10(an early marker of epidermal differentiation)and CD105(a marker of mesenchymal stem cell).Results The experimental group and control group have similar nano-scale structure with uniform shape and density as well as good mechanical properties.Experimental group was able to load and stably release the EIF for more than 15 days.After seeded onto experimental group for 7 days,the hADSCs proliferated well and expressed CK10,while the CD105 expression was down regulated.Conclusion Experimental group has good biological characteristics and can induce hADSCs into the epidermal cell linage.

    Mechanism of Adipose-Derived Stem Cells Improving the Efficacy of Heterologous Acellular Dermal Matrix Implantation by Paracrine Effect

    Objective To explore whether adipose-derived stem cells(ADSCs)can effectively exert its paracrine function in heterologous acellular dermal matrix(HADM)microenvironment and consequently improve the efficacy of HADM after implantation in vivo.Methods ADSCs were seeded on HADM and microplate.Total RNA of ADSCs from both materials were collected respectively,and qRT-PCR was used to evaluate the levels of genes associated with ADSC paracrine.In addition,HADM with and without ADSC pre-culture were implanted on the back of rats.All samples were harvested 4 weeks after operation for histological staining.Results The expression of genes with respect to inflammatory regulation(TGF-β,iNOS,TSG-6,COX-2),pro-angiogenesis(VEGF,HGF,bFGF)and stemness(SOX-2)increased when ADSCs were seeded on HADM compared to ADSCs seeded on microplate.After pre-culture,in vivo implanted ADSCs-HADM exhibited improved vascularization and mitigated inflammatory response compared to untreated HADM.Conclusion HADM can provide a beneficial microenvironment for ADSCs to exert their paracrine function.After pre-culture with ADSCs in vitro,the vascularization rate of in vivo implanted HADM is accelerated and the inflammatory response is attenuated and localized.

    Efficacy of Macromolecule Crowding on the Proliferation and Osteogenic Differentiation of Adipose-Derived Stem Cells

    Objective To explore the effect of macromolecule crowding on the proliferation and osteogenic differentiation of adipose-derived stem cells(ADSCs).Methods The macromolecular crowders Ficoll 400 and Ficoll 70 were mixed with the concentrations of 25 mg/mL and 37.5 mg/mL to produce 50 mg/mL Fc mixture for the in vitro culture of ADSCs.CCK-8 test,dsDNA kit and BCA kit were used to quantitatively determine the effect of macromolecule crowding on the proliferation capacity of ADSCs under standard conditions.And the cells were subjected to osteogenic induction at different time points.Alizarin red staining was used to detect the changes in the osteogenic differentiation ability of ADSCs.Results CCK-8 test showed that the macromolecule crowding promoted the proliferation of stem cells.dsDNA analysis and BCA quantitative detection showed that the macromolecule crowding promoted the synthesis of ADSCs DNA and proteins.In addition,calcium nodules of the osteoblasts were enhanced showed by alizarin red staining,which proved that the osteoblastic differentiation ability was improved.Conclusion The macromolecular crowding niche promotes the osteogenic differentiation and proliferation of ADSCs in vitro.

    Experimental Study of Compensatory Mechanism in the Target Muscles after Amputation of Each Nerve Roots of the Brachial Plexus

    Objective To evaluate the morphological and functional changes of motor endplate in target muscles,and to explore the distribution characteristics and compensatory mechanisms after severing each nerve root of brachial plexus.Methods One hundred and twenty male SD rats were selected and randomly divided into five groups(n=24).The cutoff model of each root of the brachial plexus was built on the right side,and the left side was set up as the normal control group.At weeks 1,4,8,and 12 after amputation of each nerve root of brachial plexus,the target muscles of experimental sides and normal sides were tested by ways of acetylcholinesterase staining and single-fiber electromyography.The data of morphological and functional changes of motor endplate in target muscles were recorded and analyzed by SPSS21.0 software.Results In the group of cervical 5th or thoracic first nerve root amputation,there was no compensatory Jitter pattern found from target muscles,and the mean value of Jitter was significantly lower than that of the control side.Significant compensatory Jitter changes occurred after cervical 6th,7th,and 8th nerve root cutoff.The mean value of Jitter increased gradually in the 1-4 weeks after the cervical 7 nerve root cut,which was significantly higher than that in the control group(P<0.05),and then returned to normal after 8-12 weeks.The Jitter of the rest of the nerve roots was significantly lower than that of the control group(P<0.05).Among those groups of brachial plexus nerve root amputation,the distribution of MCD in the target muscles showing obvious compensatory changes only was found in cervical 7th group.In the group of cervical 5th or thoracic first nerve root amputation,the target muscle motor endplate area gradually expanded,acetylcholinesterase staining became lighter than before,and motor endplate gradually disappeared.The change of motor endplate in target muscle after cervical 6th nerve root resection was almost the same as that after cervical 8th nerve root cut:most of motor endplates disappear

    Objective According to the difference physiological characteristics of mastoid tissue in patients with auricle defect,different auricle reconstruction methods were used to complete ear reconstruction,and to explore the feasibility of individualized auricle reconstruction.Methods From June 2012 to October 2016,according to the different physiological characteristics of mastoid tissue,6 cases of auricle reconstruction were performed by retroauricular skin expansion,1 case by retroauricular fasciocutaneous flap expansion,and 4 cases by Nagata II method.Autogenous costal cartilage was used as ear scaffold in all patients.No complications such as chest deformity,exposure and infection of ear scaffold cartilage were observed.Results The follow-up period ranged from 10 months to 2 years,with an average of 15 months.11 patients and their families were satisfied with the shape of the reconstructed auricle.Conclusion According to the difference of physiological characteristics of mastoid tissue,individualized selection of different surgical methods is conducive to the ideal effect of ear reconstruction.

    Research of Dynamic Contrast-Enhanced Magnetic Resonance in the Breast-Conserving Operation for Early Breast Cancer

    Objective To evaluate the role of dynamic contrast-enhanced magnetic resonance in the breast-conserving operation for early breast cancer.Methods Clinical data of 42 patients with early breast cancer were retrospectively analyzed.Pathological diagnosis was used to estimate the surgical margin or number of sentinel node.The effect of breast dynamic contrast-enhanced magnetic resonance and breast doppler ultrasonography in the breast conserving operation was analyzed and compared.Results The morphological accuracy and efficiency of preoperative evaluation in the doppler ultrasonography was lower than that in the dynamic contrast-enhanced magnetic resonance(P<0.05);The primary negative margin rate,determine rate of minimally extent and sentinel node dissection rate of the oncological excision in the dynamic contrast-enhanced magnetic resonance was better than those in the doppler ultrasonography(P<0.05).There was no significant difference between the breast dynamic contrast-enhanced magnetic resonance and pathological examination in the evaluation of tumor size(P>0.05).Conclusion The breast dynamic contrast-enhanced magnetic resonance has advantages in preoperative evaluation of breast conserving operation with good safety and feasibility,and is worthy of clinical promotion.

    Surgical Treatment of Optic Canal Stenosis Caused by Craniofacial Fibrous Dysplasia

    Objective To explore the surgical treatment strategy for optic canal stenosis caused by craniofacial fibrous dysplasia.Methods A retrospective analysis was made reviewing the 18 cases from August 2012 to September 2017.The clinical and imaging findings and the discussion of the operative methods and results were summarized.Results Seven patients were treated with optic nerve decompression,five patients received prophylactic decompression,and six patients received fibrous dysplasia resection.At the end of follow-up,among patients who received optic nerve decompression,3 patients achieved visual improvement,7 maintained previous vision and 2 had decreased visual acuity.Conclusion Optic nerve decompression can effectively terminate the trend of visual deterioration and possibly improve visual acuity for patients suffered from optic canal stenosis caused by craniofacial fibrous dysplasia.Neuronavigation can effectively improve the efficiency and safety of operation,and reduce the risk of optic nerve injury and visual deterioration.

    Application of Dynamic Infrared Thermography in Perioperative Period of Skin Flap Transplantation

    Objective To explore the application and techniques of dynamic infrared thermography device for perioperative monitoring of skin flap transplantation.Methods Thermal images were obtained by FLIR ONE pro camera,a portable thermal imager which can take infrared thermographs and optical photographs at the same time.The flaps and surrounding areas were photographed pre-operation,intra-operation and post-operation.Then the thermal images were analyzed by software FLIR Tools and the temperature of several spots or regions on the flaps were measured.At the same time,color Doppler ultrasound was also performed to locate the perforator vessels.The results of these two detecting methods were compared.A total of 16 clinical applications were performed.Results Before flap transplantation,the position of the blood vessels could be accurately located on thermal images.The skin temperature of the blood vessels running area was higher than the surrounding area,usually with the"hot spots"or"hot lines"on the thermal images.The result was confirmed by color Doppler ultrasound in all 16 cases.Intraoperative thermography could be used to determine whether the pedicle vessels were patency and observe the blood supply of distal flaps,by measuring the temperature of different regions on the flap.Postoperative thermal images were efficient in monitoring the blood supply of the flaps.By means of comparing the temperature of the flap and the surrounding normal skin,and recording the change of flap temperature over time,the flap microcirculation could be observed,which helps to detect the flap blood flow disorder early,so that interventions can be taken in time.Conclusion Dynamic infrared thermography is a non-invasive,economical,simple,intuitive and accurate auxiliary detection method.It’s practical in vascular localization and sensitive in intraoperative and postoperative blood supply monitoring.

    Effectiveness of Sirolimus for Microcystic Lymphatic Malformation in Children

    Objective To explore the efficacy of sirolimus in treatment of microcystic lymphatic malformation in children.Methods Nine children with microcystic lymphatic malformation were selected and treated with sirolimus orally.The sirolimus was taken orally with the dose of 0.05~0.10 mg/Kg·d in 1-2 times.The treatment lasted for 3-6 months and further consultation was required once a month during the treatment.Tumor size and complications were observed.Results All patients were followed up for 6-18 months.In 7 cases who received early treatment,the curative effect was good with obvious atrophy of the tumor,and in 2 cases who started treatment when they were over 10 years old,the curative effect was poor with no obvious shrinking of the tumor.No obvious abnormalities were found in blood routine,blood glucose,blood pressure,heart function,liver and kidney function.Only 1 patient had immune decline,but it was tolerable.Conclusion Sirolimus is effective and safe for the treatment of microcystic lymphatic malformation in children and early treatment is necessary.

    Effect of Pingyang Mycin on Hemangioma and Vascular Malformation in Children's Oral and Maxillofacial Region

    Objective To explore the effect of Pingyang mycin on hemangioma and vascular malformation in oral and maxillofacial region of children.Methods From January 2015 to January 2018,76 cases with oral and maxillofacial hemangioma and vascular malformation in children were randomly selected.According to the principle of single blindness,76 cases were divided into control group and treatment group.Patients in control group(n=38)were treated with dexamethasone,while patients in treatment group(n=38)were treated with pingyang mycin.The therapeutic effects and adverse reactions of the two groups were observed and compared.Results The effective rate of treatment group was significantly higher than that of control group(P<0.05).There was no significant difference in pre-treatment tumor area between the treatment group and the control group(P>0.05);After treatment,the tumor area of the two groups were both significantly reduced(P<0.05),and the tumor area of the treated group was significantly smaller than that of the control group(P<0.05).The incidence of adverse reactions in treatment group was significantly lower than that in control group(P<0.05).Conclusion Pingyang mycin is effective and safe in treating oral and maxillofacial hemangioma and vascular malformation in children,and is worthy of clinical promotion.

    Objective To explore the effect of intense pulsed photon rejuvenation combined with hyaluronic acid dressing in the treatment of facial hormone-dependent dermatitis.Methods From February 2016 to April 2018,96 patients with hormone-dependent facial dermatitis were randomly divided into 2 groups:experimental group and control group.Patients in control group received intense pulse photon rejuvenation only.Patients in experimental group were treated with hyaluronic acid dressing on the basis of intense pulse photon rejuvenation.The differences in treatment efficiency,treatment satisfaction,burn pain,disappearance time of fine wrinkles and incidence of adverse reactions between the two groups were observed.Results The effective rate and satisfaction of the experimental group were significantly higher than those of the control group(P<0.05).The disappearance time of fine wrinkles in the experimental group were significantly shorter than those in the control group(P<0.05).The incidence of adverse reactions in the experimental group was significantly lower than that of the control group(P<0.05).Conclusion The treatment of facial hormone-dependent dermatitis with hyaluronic acid on the basis of intense pulse photon rejuvenation can significantly improve the effective rate of treatment and reduce the incidence of adverse reactions.It is worthy of extensive promotion.

    Effectiveness of Dexmedetomidine Combined with Dizosin in the Anesthesia for Liposuction

    Objective To investigate the effectiveness of dexmedetomidine combined with dizosin in anesthesia for liposuction.Methods Sixty patients undergoing liposuction were reviewed and randomly divided into 2 groups:observation group(n=30)and control group(n=30).In observation group,dexmedetomidine combined with dizosin were taken for intravenous anesthesia,while midazolam combined with remifentanil were taken for intravenous anesthesia in control group.Scoring criteria included Ramsay sedation score,VAS pain score and HR,MAP,SpO2 at different time points were all observed.Recovery time and awake time of the two groups were recorded.The adverse reactions were observed.Results The respiratory rate and oxygen saturation at T1,T2,T3 and T4 in the observation group were higher than those in the control group and the heart rate and mean arterial pressure at T3,T4 were lower than those in the control group(P<0.05).The recovery time and awake time in observation group were shorter than those in control group(P<0.05).The incidence rate of postoperative restlessness and oxygen saturation<92%were both lower in the observation group than in the control group(P<0.05).Conclusion For the patients undergoing liposuction,dexmedetomidine combined with dizosin as intravenous anesthesia has good effect,it has advantages of shorter recovery and awake time,lower incidence of adverse reactions,and higher safety.

    Objective To investigate the effect of preoperative intravenous infusion of dexmedetomidine combined with ultrasound-guided lumbosacral plexus block in elderly patients undergoing hip replacement surgery.Methods Eighty elderly patients with hip fracture were treated with hip replacement surgery.They were divided into 2 groups:groupⅠand groupⅡ.Patients in groupⅠreceived general anesthesia,and patients in groupⅡreceived venous anesthesia combined with ultrasound-guided lumbosacral plexus block.The VAS scores of the two groups in 48 hours after surgery,the expression of S100-βand NSE,and the incidence of POD within 7 days after surgery were observed.Results The VAS scores of groupⅡwere significantly lower than those of groupⅠat 2,6,12,24 and 36hours after operation(P<0.05).The expressions of S100-βand NSE in groupⅡwere significantly lower than those in groupⅠat each time point after operation(P<0.05).The incidence rate of POD in groupⅡwas significantly lower than that in groupⅠ(P<0.05).Conclusion Dexmedetomidine combined with ultrasound-guided lumbosacral plexus block used in elderly patients undergoing hip arthroplasty has good analgesic effect and improved postoperative cognitive function,which is worthy of clinical promotion and application.

    Nursing Effect of Periodic Nursing Intervention in Head and Face Keloid

    Objective To explore the nursing effect of periodic nursing intervention in head and face keloid.Methods From April 2014 to April 2017,62 cases with head and face keloid were included,and were randomly divided into 2 groups:control group and study group.Patients in control group were given routine nursing intervention,while patients in study group were given periodic nursing intervention on the basis of routine nursing intervention.The nursing effect,anxiety and depression status of the two groups were evaluated.The adverse reactions and complications of the patients were statistically analyzed.Results The total effective rate of the study group and control group was 64.52%and 54.84%respectively,and the difference was statistically significant(P<0.05);The SAS and SDS scores of study group were significantly lower than that of control group(P<0.05);No adverse reaction was observed in the study group,and the adverse reaction rate in the control group was 22.6%.The difference was statistically significant(P<0.05).Conclusion The application of periodic nursing intervention in head and face keloid can alleviate psychological stress reaction,eliminate the bad feelings and increase the cosmetic effect.It is worthy of clinical promotion.

    Stromal vascular fraction(SVF)is a heterogeneous collection of cells,which is the remaining ingredients after removing mature fat cells in the adipose tissue of transplantation.SVF includes adipose-derived stem cells(ADSCs),hematopoietic stem cell,endothelial precursor cells,macrophages,etc.ADSCs have multipotent differentiation capacity and can release of various cytokines thus show promise in regenarative and reconstructive medicine.The methods for acquiring SVF mainly include enzymatic and mechanical isolation,which have not reached a wide consensus.SVF has been widely used in plastic surgery including enhancing the viability of fat grafts,skin rejuvenation,treatment of facial scars and alopecia,etc.SVF is also comparably effective in several human diseases including radiation injuries,osteoarthritis,diabetes and so on.But there is still debate with regards to safe and efficacious use of SVF.Further studies are needed to investigate the optimal procedures for efficient SVF isolation,elucidate its characterization and discover newer applications apart from the plethora already identified.

    Keloid is a kind of dermatosis with connective tissue hyperplasia.Several etiological factors for keloids have been proposed in the past,including genetic predisposition,tension present in the local skin environment and immunoendocrinology factors.However,the pathogenesis of keloid has not been elucidated,and there is no appropriate treatment to get the satisfactory therapeutic effect.At present,comprehensive treatment based on clinical manifestations is mainly applicated to obtain the best therapeutic effect.In this paper,the progress of clinical therapy and pathogenesis of keloids in recent years were reviewed.
